| Adult UTI, acute uncomplicated 100 or 250 mg orally 12 hrly for 3 days, mild/moderate 250 mg orally or 200 mg IV 12 hrly for 7-14 days, severe/complicated 500 mg orally or 400 mg IV 12 hrly for 7-14 days. Chronic bacterial prostatitis, mild/moderate 500 mg orally or 400 mg IV 12 hrly for 28 days. Lower resp tract infections, mild/moderate 500 mg orally or 400 mg IV 12 hrly for 7-14 days, severe/complicated 750 mg orally 12 hrly or 400 mg IV 8 hrly for 7-14 days. Nosocomial pneumonia, mild/moderate/severe 400 mg IV 8 hrly for 10-14 days. Acute sinusitis, mild/moderate 500 mg orally or 400 mg IV 12 hrly for 10 days. Skin & skin structure, mild/moderate 500 mg orally or 400 mg IV 12 hrly for 7-14 days, severe/complicated 750 mg orally 12 hrly or 400 mg IV 8 hrly for 7-14 days. Bone & joint infections, mild/moderate 500 mg orally or 400 mg IV 12 hrly for ≥4-6 weeks, severe/complicated 750 mg 12 hrly orally or 400 mg IV 8 hrly for ≥4-6 weeks. Complicated intra-abdominal infections 500 mg orally or 400 mg IV 12 hrly for 7-14 days. Infectious diarrhea, mild/moderate/severe 500 mg orally 12 hrly for 5-7 days. Typhoid fever, mild/moderate 500 mg orally 12 hrly for 10 days. Uncomplicated urethral & cervical gonococcal infections 250 mg orally as a single dose. Empirical therapy in severe febrile neutropenic patients 400 mg IV 8 hrly in combination w/ piperacillin 50 mg/kg 4 hrly for 7-14 days. Max: 24 g/day. Inhalational anthrax (post-exposure) 500 mg orally or 400 mg IV 12 hrly for 60 days. Childn Complicated UTI or pyelonephritis (patients 1-17 yr) 10-20 mg/kg (max: 750 mg/dose) orally 12 hrly or 6-10 mg/kg (max: 400 mg/dose) IV 8 hrly for 10-21 days. Inhalational anthrax (post-exposure) 15 mg/kg (max: 500 mg/dose) orally or 10 mg/kg (max: 400 mg/dose) IV 12 hrly for 60 days. |
