Febuxostat
Indications/Uses
Treatment of chronic hyperuricemia in conditions where urate deposition has already occurred (including a history or presence of tophus &/or gouty arthritis).
Dosage/Direction for Use
Adult 20-40 mg once daily. May be increased to 120 mg once daily if serum uric acid is >6 mg/dL after 2-4 wk. Hepatic impairment 80 mg/day.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Pregnancy & lactation.
Special Precautions
Do not use until acute gouty attacks (gout flare) has completely subsided. Not recommended for patients w/ ischemic heart disease or CHF; xanthine deposition (eg malignant disease or Lesch-Nyhan syndrome); altered thyroid function. Perform liver function test prior to treatment & periodically thereafter. Severe renal & hepatic impairment. Childn <18 yr.
Adverse Reactions
Liver function abnormalities, diarrhea, headache, nausea & rash.
Drug Interactions
May increase AUC of desipramine/CYP2D6 substrate. Delayed absorption w/ antacid-containing Mg & Al hydroxide. Mercaptopurine/azathioprine; theophylline. NSAIDs eg naproxen & probenecid. Increased metabolism & decreased efficacy w/ potent inducers of UGT enzymes.
