Imipenem 500 mg, cilastatin Na 500 mg
Indications/Uses
Treatment of documented multidrug resistant gm -ve infections due to organisms proven or suspected to be susceptible to carbapenems & for polymicrobial infections (eg mixed aerobic & anaerobic infections) in which other agents have insufficient activity or contraindicated due to toxic potential.
Dosage/Direction for Use
IV 250 or 500 mg over 20-30 min or 1 g over 40-60 min. Adult 1.5-2 g daily in 3-4 equally divided doses. May increase to a max dose of 50 mg/kg/day or 4 g daily. Moderate infection 500 mg 8 hrly. Severe infection 500 mg 6 hrly. Severe &/or life-threatening 1,000 mg 8 hrly. Infections due to less susceptible organisms 1,000 mg 6 hrly. Childn ≥3 yr >40 kg Adult dose; <40 kg 15 mg/kg 6 hrly. Max: 2 g daily.
Contraindications
Hypersensitivity to imipenem, cilastatin, penicillins, cephalosporins or other β-lactams.
Special Precautions
Not recommended for treatment of meningitis. Discontinue use if allergic reaction occurs. History of GI disease, particularly colitis. CNS disorders &/or compromised renal function. Hemodialysis should be instituted w/in 48 hr in patients w/ CrCl ≤5 mL/min. Potential risk of convulsions in patients undergoing hemodialysis. Childn ≥3 yr.
Adverse Reactions
Mild increase in serum transaminases, bilirubin &/or serum alkaline phosphatase, rash, pruritus, urticaria, erythema, local pain & induration, thrombophlebitis.
Drug Interactions
Risk of seizures w/ ganciclovir. Probenecid, valproic acid.
