Indications/Uses
Nosocomial pneumonia, community-acquired pneumonia (CAP); complicated skin & skin structure infections including diabetic foot infections w/o concomitant osteomyelitis; uncomplicated skin & skin structure infections; vancomycin-resistant Enterococcus faecium infections including cases w/ concurrent bacteremia.
Dosage/Direction for Use
Nosocomial pneumonia, CAP including concurrent bacteremia & complicated skin & skin structure infections 600 mg IV or oral every 12 hr in adult & adolescent (≥12 yr) & 10 mg/kg IV or oral every 8 hr in childn (0-11 yr) for 10-14 days. Vancomycin-resistant Enterococcus faecium infections including concurrent bacteremia 600 mg IV or oral every 12 hr in adult & adolescent (≥12 yr) & 10 mg/kg IV or oral every 8 hr in childn (0-11 yr) for 14-28 days. Uncomplicated skin & skin structure infection Adult 400 mg every 12 hr, adolescent 600 mg every 12 hr, childn 5-11 yr 10 mg/kg every 12 hr, <5 yr 10 mg/kg every 8 hr for 10-14 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Not to be used in patients taking products inhibiting monoamine oxidases A or B (eg, phenelzine, isocarboxazid) or taking any such products w/in 2 wk.
Special Precautions
Myelosuppression (including anemia, leukopenia, pancytopenia & thrombocytopenia); peripheral & optic neuropathy; serotonin syndrome; mortality imbalance in an investigational study in patients w/ catheter-related blood stream infections, including those w/ catheter-site infections; Clostridium difficile associated diarrhea; patients w/ uncontrolled HTN, pheochromocytoma, thyrotoxicosis &/or patients taking directly & indirectly acting sympathomimetic agents (eg, pseudoephedrine), vasopressive agents (eg, epinephrine & norepinephrine), dopaminergic agents (eg, dopamine, dobutamine). Lactic acidosis. Convulsions. Hypoglycemia. Development of drug-resistant bacteria. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Headache, diarrhea, nausea, vomiting.
