Insulin glargine
Dosage/Direction for Use
Individualized dose. Once a day preferably at the same time every day. May inj up to 3 hr before or after the usual time of administration.
Contraindications
Special Precautions
Hypoglycemia &/or hyperglycemia. Patients w/ illness or major injury. Kidney or liver problems. Concomitant use w/ pioglitazone. Avoid abrupt w/drawal. Switching from another type & brand of insulin. May affect ability to drive & use tools or machine. Pregnancy & lactation. Childn & adolescents <18 yr. Elderly ≥65 yr.
Adverse Reactions
Hypoglycemia. Skin & allergic reactions at inj site; reddening, pain, itching, hives, swelling or inflammation at the inj site reaction. Lipoatrophy or lipohypertrophy when inj too often at the same place.
Drug Interactions
Hypoglycemia w/ other antidiabetics, disopyramide, fluoxetine, sulfonamide antibiotics, fibrates, MAOIs, ACE inhibitors; medicines to relieve pain & lower fever eg, pentoxifylline, propoxyphene & salicylates (eg, acetylsalicylic acid); pentamidine. Hyperglycemia w/ corticosteroids (eg, cortisone), danazol, diazoxide, PIs, diuretics, glucagon, INH, somatropin, thyroid hormones; oestrogens & progestogens; clozapine, olanzapine & phenothiazine derivatives; sympathomimetics eg, epinephrine (adrenaline), salbutamol & terbutaline. Co-administration w/ β-blockers or clonidine; lithium salts. Pioglitazone, alcohol.
