Tolterodine tartrate
Indications/Uses
Symptomatic treatment of urge incontinence &/or increased urinary frequency & urgency as may occur in patients w/ overactive bladder syndrome.
Dosage/Direction for Use
2 mg bd. Patients w/ impaired liver function or severely impaired renal function (GFR <30 mL/min) 1 mg bd.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Urinary retention, uncontrolled narrow angle glaucoma, myasthenia gravis, severe ulcerative colitis, toxic megacolon.
Special Precautions
Significant bladder outlet obstruction at risk of urinary retention; GI obstructive disorder (eg, pyloric stenosis); autonomic neuropathy; hiatus hernia; risk for decreased GI motility. Congenital or documented acquired QT prolongation; electrolyte disturbances eg, hypokalemia, hypomagnesaemia, hypocalcaemia; bradycardia; relevant pre-existing cardiac diseases ie, cardiomyopathy, myocardial ischaemia, arrythmia, CHF; concomitant administration of drugs known to prolong QT-interval including Class IA & III anti-arrhythmics. Concomitant treatment w/ potent CYP3A4 inhibitors. Renal impairment; hepatic disease. May affect ability to drive & operate machine. Pregnancy & lactation. Not recommended for childn.
Adverse Reactions
Headache; dry mouth. Bronchitis; dizziness, somnolence, paresthesia; dry eyes, abnormal vision including abnormal accommodation; vertigo; palpitations; dyspepsia, constipation, abdominal pain, flatulence, vomiting, diarrhoea; dry skin; dysuria, urinary retention; fatigue, chest pain, peripheral oedema; increased wt.
Drug Interactions
Increased serum conc w/ potent CYP3A4 inhibitors (eg, macrolide antibiotics, antifungal agents & antiproteases). More pronounced therapeutic effect & side effects w/ drugs that possess antimuscarinic properties. Reduced therapeutic effect w/ muscarinic cholinergic receptor agonist. Decreased effects of prokinetics (eg, metoclopramide & cisapride).
