Indications/Uses
Dosage/Direction for Use
HTN, angina pectoris, cardiac arrhythmias, migraine prophylaxis 100-200 mg/day in single or divided doses (morning & evening). MI 200 mg daily in divided doses (morning & evening). Functional heart disorders w/ palpitations 100 mg once daily. Hyperthyroidism 150-200 mg/day in 3-4 divided doses. Elderly, renal impairment, hepatic impairment No dose adjustment necessary.
Administration
May be taken with or without food: Swallow whole, do not chew/crush.
Contraindications
2nd or 3rd AV block, marked sinus bradycardia, sick sinus syndrome, cardiogenic shock, severe peripheral arterial circulatory disorder. Acute MI.
Special Precautions
Heart failure, preexisting AV conduction disorder, bradycardia, peripheral arterial circulatory disorder, phaeochromocytoma, patient undergoing surgery, non-cardiac surgery. Abrupt w/drawal especially in patients w/ ischaemic heart disease. Concomitant use w/ Ca antagonists of verapamil type. Pregnancy & lactation. May impair ability to drive & operate machinery.
Adverse Reactions
Bradycardia, postural disorders, cold hands & feet, palpitations, fatigue, dizziness, headache, GI disturbances, thrombocytopenia, liver function test abnormalities, wt gain, CNS disorders, dyspnoea on exertion, eye irritation, skin rash.
Drug Interactions
Raised plasma levels w/ compounds metabolised by CYP2D6 eg, antiarrhythmics, antihistamines, histamine-2-receptor antagonists, antidepressants, antipsychotics & COX-2 inhibitors. Lowered plasma conc w/ alcohol & hydralazine. Sympathetic ganglion-blocking agents, other β-blockers or MAOIs, clonidine. Verapamil, diltiazem or antiarrhythmic agents. Increased AV conduction time w/ digitalis glycosides. Cardiodepressant effect may be enhanced by inhalation anaesth. Decreased effect w/ indomethacin, other prostaglandin synthetase inhibitors. Adrenaline, oral antidiabetics.
