Indications and Dosage

Topical/Cutaneous
Tinea corporis, Tinea cruris

Adult: Adult: Available preparation
Each cream or lotion contains betamethasone diproprionate 0.05% and clotrimazole 1%

Apply onto affected area(s) bid, morning and evening, for up to 2 weeks. Max: 45 g /week.
Child: ≥17 yr Same as adult dose.

Topical/Cutaneous
Tinea pedis

Adult: Adult: Available preparation
Each cream or lotion contains betamethasone 0.05% and clotrimazole 1%

Special Precautions

Not recommended for use in diaper dermatitis and under occlusion. Pregnancy and lactation.

Adverse Reactions

Hypercorticism or suppression of hypothalamic-pituitary-adrenal (HPA) axis, Cushing's syndrome, hyperglycaemia, glycosuria; dry skin, burning sensation, paraesthesia, oedema, rash, secondary infection, stinging, skin ulceration.

Action

Description: Betamethasone prevents and reduces inflammation by depressing the migration of polymorphonuclear leukocytes and fibroblasts; reversing capillary permeability and lysosomal stabilisation at the cellular level. Clotrimazole binds to 14-α-demethylation of lanosterol in fungi by binding to 1 of the cytochrome P-450 enzymes leading to the accumulation of 14-α-methylsterols which affects electron transport, and reduced conc of ergosterol which is essential for a normal fungal cytoplasmic membrane, thereby inhibiting growth of fungi.
Pharmacokinetics:
Absorption: Betamethasone: Absorbed through the skin and enters systemic circulation. Clotrimazole: Penetrates the epidermis.
Distribution: Betamethasone: Crosses the placenta.
Metabolism: Undergoes hepatic metabolism. Clotrimazole: Converted to inactive compounds.
Excretion: Betamethasone: Via urine; bile (some amount). Clotrimazole: Via faeces and urine.
