Indications/Uses
Treatment of infections of resp tract, ear nose & throat, skin; infections caused by nontubercular mycobacteria; H. pylori-induced duodenal ulcers.
Dosage/Direction for Use
Upper & lower resp tract Adult & adolescent >12 yr 250 mg bd. Serious infections: 500 mg bd. Infection by nontuberculous mycobacteria Adult & adolescent 500 mg bd. May be increased to 1,000 mg bd if no improvement after 3-4 wk. Prophylaxis: 1,000 mg in 2 separate doses. Eradication of H. pylori Adult & adolescent >12 yr Clarithromycin 500 mg, omeprazole 20 mg & either amoxicillin 1,000 mg or metronidazole 400 mg. To be taken bd for 7 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to macrolides. Concomitant therapy w/ cisapride, pimozide or terfenadine. When used w/ omeprazole: Concomitant therapy w/ terfenadine, astemizole cisapride, pimozide or carbamazepine; moderately severe to severe liver & kidney function.
Special Precautions
Liver impairment, kidney disease, serious GI problems, heart diseases, asthma.
Adverse Reactions
Hypersensitivity reactions; loss of appetite, nausea, vomiting, diarrhea, constipation, flatulence; kidney inflammation & acute renal failure; deficiencies of blood platelets; hyperactivity, anxiety, confusion, hallucinations, headache; palpitations, chest pain, irregular heartbeat; severe skin reactions; hearing impairment.
Drug Interactions
Digoxin, disopyramide, terfenadine, astemizole, theophylline, carbamazepine, ritonavir, cisapride, pimozide, lovastatin, phenytoin, ergotaminem, triazolam, midazolam or cyclosporine, tacrolimus or oral anticoagulants, zidovudine.
