Indications/Uses
Essential HTN in adults. HTN in childn & adolescents (6 to <18 yr). Heart failure & impaired left ventricular systolic function (left ventricular ejection fraction ≤40%) in adults when ACE inhibitors are not tolerated or as add-on therapy to ACE inhibitors.
Dosage/Direction for Use
HTN Adult Initially 8 mg once daily. May be increased to 16 mg once daily. Max: 32 mg once daily. Childn & adolescent 6 to <18 yr, weighing ≥50 kg Initially 4 mg once daily, can be increased to 8 mg once diaily then 16 mg if needed; weighing <50 kg Initially 4 mg once daily, can be increased to 8 mg once daily. Patients w/ intravascular vol depletion Initially 4 mg. Renal impairment Initially 4 mg. Mild to moderate hepatic impairment Initially 4 mg. Heart failure Initially 4 mg once daily. May be increased to 32 mg once daily (max dose), or achieve highest tolerated dose by doubling the dose at intervals of at least 2 wk.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2). Severe hepatic impairment &/or cholestasis. 2nd & 3rd trimester of pregnancy. Childn <1 yr.
Special Precautions
Changes in renal function. Periodically monitor serum K & creatinine levels in patients w/ renal impairment. Monitor BP in patients on haemodialysis. May increase blood urea & serum creatinine in patients w/ bilateral renal artery stenosis or stenosis of the artery to a solitary kidney. Hypotension in heart failure patients, hypertensive patients w/ intravascular vol depletion or during anaesth & surgery. Patients w/ aortic & mitral valve stenosis or obstructive hypertrophic cardiomyopathy. Not recommended for primary hyperaldosteronism. Hyperkalaemia. Patients w/ ischaemic cardiopathy or ischaemic cerebrovascular disease. Kidney transplantation. Concomitant use w/ ACE inibitor in heart failure. Concomitant use w/ ACE inhibitor, angiotensin II receptor blockers or aliskiren. Pregnancy (1st trimester) & lactation. Childn w/ GFR <30 mL/min/1.73 m2, or w/ possible intravascular vol depletion.
Adverse Reactions
Treatment of HTN: Adult & childn: Headache, dizziness, upper resp infection. Childn: Cough, rash. Treatment of heart failure: Hyperkalaemia, hypotension, renal impairment.
Drug Interactions
Increased K levels w/ K-sparing diuretics, K-supplements, salt substitutes containing K & other medicinal products eg, heparin. Dual blockade of the renin-angiotensin-aldosterone-system w/ ACE inhibitors, angiotensin II receptor blockers or aliskiren. Increased serum lithium conc. Attenuated antihypertensive effect & worsening of renal function w/ NSAID.
