Ifosfamide
Indications/Uses
Inoperable malignant tumours sensitive to ifosfamide.
Dosage/Direction for Use
Individualized dosing. 30-60 mg/kg IV over 30-120 min daily for 5 consecutive days or 125-200 mg/kg IV over 24 hr per cycle. Therapy cycles may be repeated every 3-4 wk.
Contraindications
Hypersensitivity. Severe bone marrow depression; renal impairment; bilateral urinary flow obstruction, active infections, acute haemorrhagic cystitis. Pregnancy & lactation.
Special Precautions
Patients of reproductive age should use contraceptives during therapy & for the following 6 mth. Unilateral nephrectomies; obstruction of efferent urinary tract, cystitis, brain metastases, cerebral symptoms, infection, electrolyte imbalance. Patients w/ previous radiotherapy. DM, hepatic & renal impairment. Concomitant use w/ nephrotoxic drugs. Timely administration of antiemetics. Regular monitoring of renal function, urinalysis & blood count. Ensure adequate diuresis & meticulous oral hygiene. May impair ability to drive or operate machinery. Elderly.
Adverse Reactions
Myelosuppression (leucocytopenia, thrombocytopenia & anaemia), haemorrhagic cystitis, reversible encephalopathy, nausea & vomiting, azoospermia, oligospermia, ovulation disturbances, alopecia.
Drug Interactions
Cytostatics, radiation, cisplatin, aminoglycosides, acyclovir, amphotericin B; sulfonylureas; allopurinol, hydrochlorothiazide; warfarin; chlorpromazine & disulfiram; phenobarb, phenytoin, chloral hydrate, suxamethonium. Live virus vaccine.
