Indications/Uses
Duodenal ulcer, gastric ulcer & reflux oesophagitis. In combination w/ antibiotics for the treatment of duodenal ulcer patients infected w/ Helicobacter pylori. NSAID-associated peptic ulcer & gastroduodenal erosions. Prophylactic treatment in patients w/ an increased risk of NSAID-associated peptic ulcer, gastroduodenal erosions or dyspeptic symptoms. Maintenance therapy of patients w/ chronic relapsing peptic ulcer & reflux oesophagitis. Symptomatic treatment of heartburn & regurgitation in GERD. Zollinger-Ellison syndrome.
Dosage/Direction for Use
Duodenal ulcer 20 mg once daily for 2-4 wk. Refractory cases: 40 mg daily for 4 wk. H. pylori eradication, associated duodenal ulcer Triple therapy regimens: Losec MUPS 20 mg, amoxicillin 1 g & clarithromycin 500 mg all bd for 1 wk; or Losec MUPS 20 mg, clarithromycin 250 mg & metronidazole 400 mg all bd for 1 wk. Dual therapy regimens: Losec MUPS 40 mg once daily w/ clarithromycin 500 mg tds for 2 wk; or Losec MUPS 20 mg bd w/ amoxicillin 750-1,000 mg bd for 2 wk. Gastric ulcer/reflux oesophagitis 20 mg once daily for 4-8 wk. Refractory cases: 40 mg once daily for 8 wk. NSAID-associated gastric ulcers or gastric erosions, duodenal ulcers or duodenal erosions, w/ or w/o concomitant NSAID treatment 20 mg once daily for 4-8 wk. Prevention of NSAID-associated gastric ulcer, gastric erosions, duodenal ulcers, duodenal erosions or dyspeptic symptoms 20 mg once daily. Prevention of relapse in patients w/ recurrent duodenal ulcer refractory to H. pylori eradication treatment regimens, prevention of relapse in patients w/ recurrent reflux oesophagitis 10-40 mg once daily. Maintenance therapy for gastric ulcer 20-40 mg once daily. Symptomatic treatment of GERD 10-20 mg once daily. Ulcer-like symptoms 10-20 mg once daily. Zollinger-Ellison syndrome Initially 60 mg once daily. If >80 mg/day is required, it should be in 2 divided doses. Hepatic impairment 10-20 mg daily. Elderly, renal impairment No dose adjustment needed.
Administration
May be taken with or without food: Swallow whole w/ at least ½ glass of fluid. Do not chew/crush. May also be dispersed in water or a slightly acidic fluid eg, fruit juice. Drink mixt w/in 30 min.
Contraindications
Hypersensitivity to substituted benzimidazoles. Concomitant use w/ atazanavir.
Special Precautions
Exclude malignancy if gastric ulcer is suspected. Co-administration w/ atazanavir, clopidogrel, methotrexate. May reduce the absorption of vit B12. Fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency. Slightly increased risk of GI infections. Hypomagnesemia. Increased risk for osteoporosis-related fractures of the hip, wrist or spine.
Adverse Reactions
Headache, abdominal pain, constipation, diarrhoea, flatulence & nausea/vomiting.
Drug Interactions
Decreased plasma levels of nelfinavir & atazanavir. Increased bioavailability of digoxin. Decreased exposure to the active metabolite of clopidogrel. Reduced absorption of posaconazole, erlotinib, ketoconazole & itraconazole. Increased systemic exposure to active substances metabolised by CYP2C19 (eg, R-warfarin & other vit K antagonists, cilostazol, diazepam & phenytoin). Increased Cmax & AUC of cilostazol. Monitor phenytoin plasma conc. Increased plasma levels of saquinavir/ritonavir, tacrolimus. May lead to increased omeprazole serum levels w/ CYP2C19 or CYP3A4 inhibitors (eg, clarithromycin & voriconazole). May lead to decreased omeprazole serum levels w/ CYP2C19 or CYP3A4 inducers or both (eg, rifampicin & St. John’s wort).
