Indications/Uses
Infections in adult & childn >3 mth: Severe pneumonia (eg, hospital & ventilator-associated pneumonia); broncho-pulmonary infections in cystic fibrosis; complicated UTI & intra-abdominal infections; intra- & post-partum infections; complicated skin & soft tissue infections; acute bacterial meningitis. Bacteraemia in association w/, or is suspectedly associated w/ any of the infections previously mentioned. Management of neutropenic patients w/ fever suspectedly due to bacterial infection.
Dosage/Direction for Use
Adult & adolescents Severe pneumonia (eg, hospital & ventilator-associated pneumonia); complicated UTI & intra-abdominal infections; intra- & post-partum infections; complicated skin & soft tissue infections 500 mg or 1 g every 8 hr by IV infusion over approx 15-30 min, or as IV bolus inj over approx 5 min. Febrile neutropenia 1 g every 8 hr by IV infusion over approx 15-30 min, or as IV bolus inj over approx 5 min. Broncho-pulmonary infections in cystic fibrosis, acute bacterial meningitis 2 g every 8 hr by IV infusion over approx 15-30 min. Childn 3 mth-11 yr & up to 50 kg body wt Severe pneumonia (eg, hospital & ventilator-associated pneumonia); complicated UTI & intra-abdominal infections; intra- & post-partum infections; complicated skin & soft tissue infections 10 or 20 mg/kg every 8 hr by IV infusion over approx 15-30 min, or as IV bolus inj over approx 5 min. Febrile neutropenia 20 mg/kg every 8 hr by IV infusion over approx 15-30 min, or as IV bolus inj over approx 5 min. Broncho-pulmonary infections in cystic fibrosis, acute bacterial meningitis 40 mg/kg every 8 hr by IV infusion over approx 15-30 min. Childn >50 kg Adult dose. Renal impairment, CrCl 26-50 mL/min 1 unit dose every 12 hr, CrCl 10-25 mL/min ½ unit dose every 12 hr, CrCl <10 mL/min ½ unit dose every 24 hr.
Contraindications
Hypersensitivity to meropenem, other carbapenem antibacterial agents & other types of β-lactam antibacterial agents (eg, penicillins or cephalosporins).
Special Precautions
History of hypersensitivity to β-lactams; discontinue if allergic reactions occur. Reports of antibiotic-associated colitis & pseudomembranous colitis; seizures. False +ve result in direct or indirect Coombs test. Concomitant use w/ valproic acid/Na valproate/valpromide is not recommended. Use w/ caution in patients on controlled Na diet. May affect ability to drive & use machines. Dose adjustment is recommended for patients w/ moderate & severe renal impairment. Monitor hepatic function during treatment due to risk of hepatic toxicity. Pregnancy & lactation. Childn <3 mth.
Adverse Reactions
Diarrhoea, rash, nausea/vomiting & inj site inflammation. Thrombocythaemia; headache; abdominal pain; increased transaminases, blood alkaline phosphatase & blood lactate dehydrogenase; pruritus; pain.
Drug Interactions
Increased elimination t½ & plasma conc w/ probenecid. Decreased blood levels of valproic acid/Na valproate/valpromide. Increased effects of oral anticoagulants (eg, warfarin).
