Indications/Uses
Treatment of essential HTN in adults whose BP is inadequately controlled on telmisartan alone.
Dosage/Direction for Use
Adult 1 tab once daily. Mild to moderate hepatic impairment Max: One 40/12.5 mg tab once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to telmisartan & hydrochlorothiazide or other sulphonamide-derived substances. Cholestasis & biliary obstructive disorders. Refractory hypokalaemia, hypercalcaemia. Concomitant use of aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2). Severe hepatic & renal (CrCl <30 mL/min) impairment. Pregnancy (2nd & 3rd trimesters).
Special Precautions
Renovascular HTN; intravascular hypovolaemia; dual blockade of renin-angiotensin-aldosterone system (RAAS); other conditions w/ RAAS stimulation; primary aldosteronism; aortic or mitral valve stenosis, or obstructive hypertrophic cardiomyopathy; metabolic & endocrine effects; electrolyte imbalance (eg, hypokalaemia, hyperkalaemia, hyponatraemia & hypochloraemic alkalosis, hypercalcaemia, hypomagnesaemia); acute myopia & angle-closure glaucoma; non-melanoma skin cancer. Risk of MI or stroke in case of excessive BP reduction in patients w/ ischaemic cardiopathy or ischaemic CV disease. Reports of exacerbation or activation of SLE w/ thiazide diuretics. Intolerance to sorbitol & lactose monohydrate. Hypersensitivity & photosensitivity reactions. Caution in patients w/ impaired hepatic function; renal impairment & kidney transplantation. Not recommended during the 1st trimester of pregnancy. Ethnic differences. Childn <18 yr.
Drug Interactions
Lithium; medicinal products associated w/ K loss & hypokalaemia; medicinal products that may increase K levels or induce hyperkalaemia; medicinal products affected by serum K disturbances; digitalis glycosides; digoxin; other antihypertensive agents; antidiabetic medicinal products (oral agents & insulin); metformin; cholestyramine & colestipol resins; NSAIDs; pressor amines (eg, noradrenaline); nondepolarizing skeletal muscle relaxants (eg, tubocurarine); medicinal products used in the treatment for gout; Ca salts; β-blockers & diazoxide; anticholinergic agents; amantadine; cytotoxic agents.
