Losartan K
HTN; diabetic nephropathy in type 2 diabetes followed w/ HTN & proteinuria.
Dosage/Direction for Use
HTN 25-50 mg once daily. Max: 100 mg/day. Diabetic nephropathy in type 2 diabetes followed w/ HTN & proteinuria 50 mg once daily, can be increased up to 100 mg/day. Patients at risk of an excessive decrease in BP Start administration from 25 mg.
May be taken with or without food.
Hypersensitivity. Patients w/ severe hepatic disorder. Concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2). Pregnancy.
Special Precautions
Patients w/ unilateral or bilateral renal artery stenosis; hyperkalemia; severe renal function disorder; history or current hepatic function disorder; cerebrovascular disorders; decreased bodily fluids (being administered w/ antihypertensive diuretic, under severe limitation of salt intake, undergoing hemodialysis). Dual blockade of the renin-angiotensin-aldosterone system (RAAS) w/ combined use of ACE inhibitors, angiotensin II receptor blockers or aliskiren. BP should be monitored regularly. Administration w/in the 24 hr before surgery is undesirable. Anemia & increased serum K & serum creatinine may occur more easily in patients w/ diabetic nephropathy in type 2 diabetes. Discontinue in case of development of hepatitis & other severe hepatic disorders. Use w/ caution when operating hazardous machinery or driving a car. Discontinue during lactation. Elderly.
Adverse Reactions
Anaphylaxis; angioedema eg, swelling of the face, lips, pharynx, tongue; acute hepatitis or fulminant hepatitis; renal failure; shock, fainting, loss of consciousness; rhabdomyolysis; hyperkalemia; arrhythmia; pancytopenia, leucopenia & thrombopenia; hypoglycemia; hyponatremia.
Drug Interactions
Increase serum K w/ K-sparing diuretic (eg, spirinolactone, triamterene), K supplement (eg, KCl), ACE inhibitor. Increased risk of hypotension, hyperkalaemia, & decreased renal function w/ ACE inhibitors, angiotensin II receptor blockers or aliskiren. Weakened antihypertensive effect w/ NSAIDs (eg, indomethacin). Increased conc of lithium leading to lithium toxicity.
