Per L OliClinomel N4-550E Lipid emulsion (80% olive oil + 20% soybean oil) 20 g, glucose 80 g, amino acids 22 g, nitrogen 3.6 g, electrolytes. Energy: 610 kCal; osmolarity: 750 mOsmol. Per L OliClinomel N7-1000E Lipid emulsion (80% olive oil + 20% soybean oil) 40 g, glucose 160 g, amino acids 40 g, nitrogen 6.6 g, electrolytes. Energy: 1,200 kCal; osmolarity: 1,450 mOsmol
Indications/Uses
Parenteral nutrition for adults & childn >2 yr when oral or enteral nutrition is impossible, insufficient or contraindicated.
Dosage/Direction for Use
Individualized dosing. OliClinomel N4-550E Adult 25-40 kCal/kg/day. Max: 40 mL/kg/day (equiv to amino acids 0.88 g, glucose 3.2 g & lipids 0.8 g/kg) ie 2,800 mL of emulsion for infusion for a 70-kg patient. Childn >2 yr 60-110 kCal/kg/day. Max: 100 mL/kg/day (equiv to amino acids 2.2 g, glucose 8 g & lipids 2 g/kg body wt). To be administer IV via central or peripheral vein. Max infusion rate: 3 mL/kg/hr. OliClinomel N7-1000E Adult 25-40 kCal/kg/day. Max: 36 mL/kg (equiv to amino acids 1.44 g, glucose 5.76 g & lipids 1.44 g/kg) ie 2,520 mL of emulsion for infusion for a 70-kg patient. Childn >2 yr 60-110 kCal/kg/day. Max: 75 mL/kg/day (equiv to amino acids 3 g, glucose 12 g & lipids 3 g/kg body wt). Do not exceed doses of 3 g/kg/day of amino acids &/or 17 g/kg/day of glucose &/or 3 g/kg/day of lipids except in particular cases. To be administer IV via central vein. Do not administer through a peripheral vein. Max infusion rate: 1.5 mL/kg/hr.
Contraindications
Premature neonates, infant & childn <2 yr. Known hypersensitivity to egg or soya proteins. Severe renal & hepatic insufficiency; high & pathological electrolyte plasma conc; congenital abnormalities of amino acid metabolism; severe blood coagulation disorders, severe hyperlipidaemia; hyperglycaemia.
Special Precautions
Water & electrolyte equilibration & metabolic disorders; increased osmolarity, adrenal insufficiency, heart failure or pulmonary dysfunction. DM, coagulation disorders, anaemia, hyperlipidemia. Pregnancy & lactation. Regularly check serum triglyceride conc (should not exceed 3 mmol/L).
Adverse Reactions
Hyperthermia, excessive sweating, tremors, nausea, headache, dyspnoea. Transient rises in liver function parameters.
Drug Interactions
Must not be administered simultaneously w/ blood through the same infusion tubing.
