Symptomatic trichomoniasis in females & males when the presence of trichomonad has been confirmed by appropriate laboratory procedures. Acute intestinal amoebiasis (amoebic dysentery) & amoebic liver abscess. Serious infections caused by susceptible anaerobic bacteria. Intra-abdominal infections, including peritonitis, intra-abdominal abscess & liver abscess, caused by Bacteroides species including the B. fragilis group, Clostridium spp, Eubacterium spp, Peptococcus niger or Peptostreptococcus spp. Skin & skin structure infections caused by Bacteroides spp including the B. fragilis group, Clostridium spp, Peptococcus niger, Peptostreptococcus spp or Fusobacterium spp. Gynaelogic infections including endometritis, endomyometritis, tubo-ovarian abscess & postsurgical vag cuff infection caused Bacteroides spp including the B. fragilis group, Clostridium spp, Peptococcus niger or Peptostreptococcus spp. Bacterial septicaemia caused by Bacteroides spp including the B. fragilis group or Clostridium spp. Bone & joint infections (as adjunctive therapy), CNS infections & endocarditis caused by Bacteroides spp including the B. fragilis group.
Dosage/Direction for Use
Amoebiasis, balantidiasis, Blastocystis hominis infection Adult 400-800 mg tds for 5-10 days. Childn 35-50 mg/kg daily in divided doses. Giardiasis Adult 2 g daily as single dose for 3 successive days. Childn 15 mg/kg daily in divided doses. Trichomoniasis Adult Single 2 g dose or 7-day course of 200 mg tds or 400 mg bd or 2-day course of 800 mg in the morning & 1.2 g in the evening. Childn 15 mg/kg daily in divided doses for 7 days. Bacterial vaginosis Adult Single 2 g dose or 7-day course of 400 mg bd. Acute necrotising ulcerative gingivitis, acute dental infections 200 mg tds for 3 days. Anaerobic bacterial infections Adult Initially 800 mg followed by 400 mg every 8 hr for 7 days. Childn 7.5 mg/kg every 8 hr.
Should be taken with food.
Hypersensitivity to metronidazole or other nitroimidazole derivatives. Patients w/ trichomoniasis during 1st trimester of pregnancy.
Special Precautions
Convulsive seizures & peripheral neuropathy; discontinue on appearance of abnormal neurologic signs. Known or previously unrecognised candidiasis may present more prominent symptoms during therapy. Patients w/ active CNS disease including epilepsy; history of blood dyscrasias; severe hepatic disease. 2nd & 3rd trimesters of pregnancy in trichomoniasis. Not recommended in nursing mothers. Breast-feeding may be resumed 24 & 48 hr after treatment is completed. Elderly.
Adverse Reactions
Convulsive seizures, peripheral neuropathy, dizziness, vertigo, incoordination, ataxia, confusion, irritability, depression, weakness, insomnia; nausea, headache, anorexia, vomiting, diarrhoea, epigastric distress, abdominal cramping, constipation, metallic taste, furry tongue, glossitis, stomatitis; reversible neutropenia (leucopenia); T-wave flattening; urticaria, erythematous rash, flushing, nasal congestion, dry mouth (or vagina or vulva), fever; dysuria, cystitis, polyuria, incontinence, sense of pelvic pressure; proliferation of Candida in the vagina, dyspareunia, decreased libido, proctitis, fleeting joint pains; taste modification of alcoholic beverages.
Drug Interactions
Disulfiram-like effect w/ alcohol. Potentiated effect of anticoagulants, coumarin or indandione-derivatives. Confusion & psychotic reactions w/ disulfiram in alcoholic patients. Decreased hepatic metabolism, delayed elimination & increased serum conc w/ cimetidine. Increased metabolism w/ phenobarb due to induction of microsomal liver enzymes. Impaired clearance of phenytoin & lithium.
