Itraconazole
Dosage/Direction for Use
Onychomycosis Pulse treatment 200 mg bd for 1 wk followed by a 3-wk drug-free interval. 2 pulses for fingernail infections & 3 pulses for toenail infections. Tinea pedis, tinea manus 200 mg bd for 7 days. Vulvovag candidiasis 200 mg bd for 1 day or 200 mg once daily for 3 days. Pityriasis versicolor 200 mg once daily for 7 days. Oral candidiasis 100 mg daily for 15 days. Fungal keratitis 200 mg once daily for 21 days. Dermatomycosis 200 mg daily for 7 days. Systemic mycoses: Aspergillosis 200 mg daily for 2-5 mth. Candidosis 100-200 mg daily for 3 wk-7 mth. Non-meningeal cryptococcosis 200 mg daily for 2 mth-1 yr. Cryptococcal meningitis 200 mg bd for 2 mth-1 yr. Histoplasmosis 200 mg once-bd for 8 mth. Sporotrichosis 100 mg daily for 3 mth. Paracoccidioidomycosis 100 mg daily for 6 mth. Chromomycosis 100-200 mg daily for 6 mth. Blastomycosis 100 mg daily-200 mg bd for 6 mth.
Administration
Should be taken with food: Take immediately after a full meal.
Contraindications
Pregnancy, hypersensitivity, terfenadine, astemizole, mizolastine, cisapride, dofetilide, quinidine, pimozide, simvastatin, lovastatin, triazolam & oral midazolam.
Special Precautions
Lactation, history of hepatic or renal disease, childn, decreased gastric acidity, CHF or other cardiac disease, significant pulmonary disease, renal failure & other oedematous disorders. Monitor hepatic function.
Adverse Reactions
GI disturbances, headache, oedema, CHF & pulmonary oedema.
Drug Interactions
Rifampicin & phenytoin reduce oral bioavailability of itraconazole. Inhibits metabolism of warfarin, digoxin, terfenadine, cyclosporin A, astemizole, mizolastine, cisapride, dofetilide, quinidine, pimozide, oral midazolam & triazolam, simvastatin, lovastatin, Ca-channel blockers. Oral anticoagulants, HIV-protease inhibitors, vinca alkaloids, busulphan, docetaxel, trimetrexate, cyclosporin, tacrolimus, rapamycin.
