FC tab Usual recommended dose: 250 mg twice daily, may be increased to 500 mg twice daily for severe infections. Duration of therapy: 7-14 days. Patient w/ renal impairment (CrCl <30 mL/min) Dose reduced by ½ & not to be continued beyond 14 days. Haemolytic streptococcal infectionAdministered at least 10 days. Dry syrChildn 7.5 mg/kg twice daily for 5-10 days. Dose may be increased to a max of 500 mg for severe infections. Streptococcal pharyngitis Childn 30-40 kg 2 mL, 20-29 kg 1.5 mL, 12-19 kg 1 mL, 8-11 kg 0.5 mL. MR tab 500-1,000 mg once daily for 7-14 days. Abbotic XLAdult 500 mg once daily. May be increased to 1,000 mg once daily.
Pseudomembraneous colitis, antibiotic-associated colitis. Hepatic or moderate to severe renal impairment. Consider management w/ fluids, electrolytes, protein supplementation. Discontinue clarithromycin if superinfection occurs. Attention to the possibility of cross resistance w/ other macrolide drugs, as well as lincomycin & clindamycin. Pregnancy & lactation. Childn <6 mth. Elderly.
