Ciprofloxacin (Tab:HCl, XR tab:HCl & betaine, IV infusion:lactate)
Indications/Uses
Tab: UTI, urethritis and cervicitis gonorrhea, GIT including typhoid fever & parathypoid, resp tract infections, skin & soft tissue infections, bone & joints infections. XR tab: Uncomplicated UTI (acute cystitis), complicated UTI, acute uncomplicated pyelonephritis. IV: Lower resp tract w/ pneumonia, kidneys &/or urinary tract, genital organs (including gonorrhea, prostatitis), abdominal cavity (GIT including typhoid & paratyphoid, biliary tract, peritonitis), skin & soft tissues, bones & joints; sepsis.
Dosage/Direction for Use
Ciproxin 250-750 mg orally 2 times daily or 200-400 mg IV 2 times daily every 12 hr or 3 times daily every 8 hr. Acute gonorrhoea A single dose of 250 mg orally or 100 mg IV. GIT infection 500 mg 2 times daily. Reduce dose if CrCl <20 mL/min. Ciproxin XR Uncomplicated UTI (acute cystitis) 500 mg once daily for 3 days. Complicated UTI & acute uncomplicated pyelonephritis 1,000 mg once daily for 7-14 days.
Administration
May be taken with or without food: May be taken w/ meals to minimise GI discomfort. Do not take w/ antacids, Fe or dairy products. XR tab: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity to ciprofloxacin or other quinolones. Pregnancy & lactation. Childn, adolescents.
Special Precautions
Epilepsy, CNS damage. Not suitable for infections that might be due to gram +ve or anaerobic pathogens.
Adverse Reactions
Nausea, diarrhea; inj & infusion site reactions (IV).
Drug Interactions
Mineral-containing antacids reduce absorption of ciprofloxacin. May increase theophylline plasma levels. Concomitant intake w/ NSAIDs can lead to convulsion. Transient rise of serum creatinine w/ cyclosporin. May augment anticoagulant effects w/ vit K antagonist. Increase tmax & AUC of sildenafil. Ropinirole, lidocaine, clozapine.
