Doripenem
Indications/Uses
Infections in adult patients eg, nosocomial pneumonia (including pneumonia due to ventilator use). Complicated intra-abdominal infections.
Dosage/Direction for Use
Nosocomial pneumonia (including pneumonia due to ventilator use) 500 mg 8 hrly for 1 or 4 hr. Complicated intra-abdominal infections 500 mg 8 hrly for 1 hr. Treatment duration: 5-14 days depending on the severity of the disease. Patient w/ renal impairment CrCl 30-50 mL/min 250 mg 8 hrly, CrCl <30 mL/min 250 mg 12 hrly.
Contraindications
Hypersensitivity to doripenem & other antibacterial carbapenems. Severe hypersensitivity reactions (eg, anaphylactic & severe skin reactions) to all other types of β-lactam antibacterials (eg, penicillin or cephalosporin).
Special Precautions
Hypersensitivity reactions. Seizure, pseudomembranous colitis, superinfection, pneumonitis. May affect ability to drive & operate machines. Pregnancy & lactation.
Adverse Reactions
Oral candidiasis, vulvomycotic infection; hypersensitivity reactions; headache; phlebitis; nausea, diarrhea, colitis due to C. difficile; liver enzyme elevation; rash, itches.
Drug Interactions
Carbapenem may reduce serum valproic acid levels. Decreased renal clearance w/ probenecid.
