Indications/Uses
Short-term treatment of post-op pain.
Dosage/Direction for Use
40 mg IM/IV followed by 20 or 40 mg 6-12 hrly as required. Max: 80 mg/day. Elderly ≥65 yr, <50 kg & moderate hepatic impairment ½ usual dose. Max: 40 mg/day.
Contraindications
Hypersensitivity to parecoxib Na, sulphonamides or to any of the excipients. Patients who have experienced bronchospasm, acute rhinitis, nasal polyps, angioneurotic oedema, urticaria or allergic-type reactions after taking ASA or NSAIDs or other COX-2 selective inhibitors. Severe hepatic impairment, active peptic ulceration or GI bleeding, inflammatory bowel disease, severe CHF. Treatment of post-op pain immediately following CABG surgery. Pregnancy (3rd trimester) & lactation.
Special Precautions
Concomitant use w/ nonspecific NSAIDs. Increased risk of serious CV thrombotic events, MI & stroke which can be fatal. CABG surgery, renal impairment, HTN, compromised cardiac/hepatic function, pre-existing edema, fluid retention, dehydration. Risk of GI ulceration, bleeding & perforation. CHF & edema. Childn <18 yr.
Adverse Reactions
Nausea; abdominal pain, constipation, dyspepsia, vomiting, edema peripheral, alveolar osteitis, hypoaesthesia, dizziness, insomnia, post-op anemia, sweating increased, pruritus, oliguria, hypotension.
Drug Interactions
ACE inhibitors, ASA, furosemide, lithium, cyclosporine, methotrexate & warfarin.
