Venlafaxine HCl
Indications/Uses
Depression including depression w/ associated anxiety. Anxiety or generalized anxiety (including long-term treatment), social anxiety & panic disorders. Prevention of relapse & recurrence of depression.
Dosage/Direction for Use
Starting Dose: 75 mg once daily. Patients not responding to initial dose may increase to max 225 mg daily. More severely depressed patients Mean dose: 350 mg daily (range 150-375 mg daily). Dose increases can be made at intervals of ≥2 wk but not <4 days. Panic disorder Initial single dose: 37.5 mg daily for 4-7 days followed by 75 mg daily to max 225 mg daily. Increments of up to 75 mg daily, as needed, w/ interval of not <4 days. Renal impairment Reduce dose by 25-50% in patient w/ GFR 10-70 mL/min. Total daily dose reduced by 50% in hemodialysis. Hepatic impairment Reduce dose by 50% in patients w/ mild to moderate hepatic impairment.
Administration
Should be taken with food: Take at approx the same time each day. Swallow whole, do not divide/crush/chew or place in water.
Contraindications
Hypersensitivity. Concomitant use w/ MAOIs.
Special Precautions
Monitor & closely observe for clinical worsening & suicidality. Discontinuation effects. Symptom of sexual dysfunction. Increased risk of bone fractures. NMS-like reactions. Patients w/ history of seizure, drug abuse, aggression. Discontinue use if seizures develop. Recent history of MI or unstable heart disease. BP monitoring is advisable in patients taking >200 mg daily doses. Control pre-existing HTN prior to treatment. Patients whose underlying conditions may be compromised by increased BP or heart rate; risk factors for QTc prolongation. Increased IOP or patients at risk for acute narrow-angle glaucoma. History or family history of bipolar disorder. Hyponatremia. Patients predisposed to bleeding, including patients on anticoagulants & platelet inhibitors. Concomitant use w/ wt loss agents. Consider measurement of serum cholesterol levels during long-term treatment. Dose tapering is recommended. May impair ability to drive or operate hazardous machinery. Pregnancy. Not recommended in nursing women. Childn <18 yr. Elderly.
Adverse Reactions
Insomnia; headache, dizziness, sedation; nausea, dry mouth, constipation; hyperhidrosis. Decreased appetite; abnormal dreams, nervousness, decreased libido, agitation, anorgasmia; akathisia, tremor, paraesthesia, dysguesia; visual impairment, accommodation disorder, mydriasis; tinnitus; tachycardia, palpitations; HTN, hot flush; dyspnoea, yawning; diarrhoea, vomiting; rash, pruritus, night sweats; hypertonia; urinary hesitation & retention, pollakiuria; erectile dysfunction ejaculation disorder; fatigue, asthenia, chills; decreased/increased wt.
Drug Interactions
Severe AR w/ MAOIs. CNS-active drugs; other serotonergic agents (including triptans, SSRIs, other SNRIs, amphetamines, lithium, sibutramine, fentanyl & its analogues, dextromethorphan, tapentadol, meperidine, methadone, pentazocine, tramadol or St. John's wort), drugs which impair serotonin's metabolism (eg, linezolid & methylene blue) or serotonin precursors (eg, tryptophan supplements). Increased risk of QTc prolongation &/or ventricular arrhythmias w/ some antipsychotics & antibiotics. Decreased AUC & Cmax of indinavir. Increased AUC & Cmax of haloperidol. Higher plasma conc w/ ketoconazole. Increased plasma conc & reduced BP-lowering effect of metoprolol. Increased AUC of risperidone. Reduced metabolism w/ CYP2D6 inhibitors. Increased levels w/ CYP3A4 inhibitors. False +ve urine immunoassay screening tests for PCP & amphetamine.
