Fluconazole
Indications/Uses
Cryptococcal meningitis, systemic & oropharyngeal candidiasis, acute & relapse vag candidiasis.
Dosage/Direction for Use
Cryptococcocal meningitis Day-1: 400 mg as a single dose, Day-2 onwards: 200-400 mg daily. Systemic candidiasis Day-1: 400 mg as a single dose, then 200 mg daily. Dose may be increased to 400 mg daily. Oropharyngeal candidiasis 50 mg once daily for 1-2 wk. If necessary, can be continued for maintenance. Max dose: 100 mg. Vag candidiasis 150 mg as a single oral dose.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to fluconazole or related azole compd.
Special Precautions
AIDS. Superinfection caused by insensitive candida strain. Discontinue if bullous lesions or erythema multiforme develop. Anaphylaxis. Hepatic or renal impairment. Pregnancy & lactation. Childn <16 yr.
Adverse Reactions
Nausea, abdominal pain, diarrhea, dyspepsia, dizziness, taste perversion, flatulence. Rash, headache. Anaphylaxis. Kidney function abnormalities, seizures, leucopenia.
Drug Interactions
Increase plasma conc of phenytoin, sulfonylurea, cyclosporin, nortriptyline, tributin, tacrolimus & zidovudine & decrease toxic metabolite production of sulfametoxazole. Decrease plasma conc w/ rifampicin. Can cause ECG abnormality w/ terfenadine. Risk in cardiac arrhythmia w/ astemizole, cisapride & terfenadine. Can increase effects of anticoagulants & decrease theophylline clearance. Prolonged serum t½ w/ sulfonylureas (eg, chlorpropamide, glibenclamide, glipizide & tolbutamide).
