Metformin HCl
Indications/Uses
Initial therapy for newly diagnosed maturity-onset diabetes in whom diet has failed. Single therapy in primary or secondary sulfonylurea failure. Combined therapy w/ sulfonylurea. Adjuvant therapy in IDDM.
Dosage/Direction for Use
Initially 1 tab twice daily. Maintenance: 1 tab 3 times daily.
Administration
Should be taken with food.
Contraindications
Diabetic coma, ketoacidosis, renal impairment, chronic liver disease, cardiac failure & recent MI; alcoholism, hypoxemia, history of lactic acidosis, shock, pulmonary insufficiency.
Special Precautions
Conditions which cause dehydration, patients w/ serious infection or trauma. Stop therapy 2-3 days before surgery or clinical investigation. Regular renal monitoring is needed.
Adverse Reactions
GI disturbances; lactic acidosis (rare).
Drug Interactions
Adjust dose of anticoagulants. Reduced renal clearance w/ cimetidine. May cause hypoglycemia w/ sulfonylurea or insulin. Risk of lactic acidosis increased by alcohol. May impair vit B12 absorption.
