Indications/Uses
Acute coronary syndrome (unstable angina or non-Q-wave MI). Percutaneous coronary intervention (PCI) including those undergoing intracoronary stenting.
Dosage/Direction for Use
Acute coronary syndrome w/ serum creatinine <2 mg/dL 180 mcg/kg IV bolus as soon as possible after diagnosis, followed by a continuous infusion of 2 mcg/kg/min up to 72 hr or until discharge. If PCI is performed during Integrilin therapy, the infusion should be continued for up to 18-24 hr after the procedure or until discharge, allowing 96-hr therapy, 2-4 mg/dL 180 mcg/kg IV bolus as soon as possible after diagnosis, followed by a continuous infusion of 1 mcg/kg/min. PCI w/ serum creatinine <2 mg/dL 180 mcg/kg IV bolus immediately before start of PCI, followed by a continuous infusion of 2 mcg/kg/min & a 2nd 180-mcg/kg bolus 10 min after the 1st bolus, continue infusion for up to 18-24 hr or until discharge, 2-4 mg/dL 180 mcg/kg IV bolus immediately before start of PCI, followed by a continuous infusion of 1 mcg/kg/min & a 2nd 180-mcg/kg bolus administered 10 min after the 1st bolus.
Contraindications
Recent stroke, major surgery, bleeding diathesis, thrombocytopenia, severe HTN, hepatic impairment, dependency on renal dialysis, prothrombin time >1.2 times control or INR ≥2, concomitant or planned administration of GP IIb/IIIa inhibitor.
Special Precautions
If severe bleeding occurs, discontinue therapy. Hepatic & renal impairment. Pregnancy & lactation.
Adverse Reactions
Bleeding, hypotension, anaphylaxis, rash, urticaria.
Drug Interactions
Increased risk of bleeding w/ heparin, oral anticoagulants, dextran soln, adenosine, sulfinpyrazone, prostacyclin, NSAIDs, ticlopidine, clopidogrel. Not compatible w/ furosemide.

B01AC16 - eptifibatide ; Belongs to the class of platelet aggregation inhibitors excluding heparin. Used in the treatment of thrombosis.
