Valproic acid.
Indications/Uses
As sole agent or adjunctive treatment of partial epilepsy (simple and complex epilepsy), absence seizure (petit mal).
Dosage/Direction for Use
For oral use.
Initial Dose: 15 mg/kg daily orally. May be increased with increment of 5-10 mg/kg daily at 1-week interval until seizure can be controlled or no aggravation of side effects. Maximum Dose: 60 mg/kg daily. Total Daily Dose: >250 mg should be given in some divided doses.
Contraindications
Patients with liver disease or significant liver dysfunction, urea cyclic disorders.
Special Precautions
Patients with a history of liver disease, using ≥2 anticonvulsants. High risk of hepatotoxicity in patients with congenital metabolic disorders, severe seizure with mental retardation or organic mental disturbances (organic brain disease). Effects on the ability to drive or operate machinery. Pregnancy and lactation. Children <2 years.
Adverse Reactions
Nausea, vomiting, gastrointestinal disturbances; sedation, tremor, ataxia, headache, nystagmus, diplopia, asterixia, spots before eyes, disarthria, dizziness, incoordination; hair loss (temporary), skin rash, erythema multiforme; emotional disorders, depression, psychosis, aggresion, hyperactive, vandalism; musculoskeletal weakness; thrombocytopenia, petechiae, bruise, hematoma, hemorrhage, limphocytosis, hypofibrinogenemia, leucopenia, eosinophilia, anemia, bone marrow suppression; increased serum transaminase level and lactate dehydrogenase, bilirubin, other abnormalities result of liver function test; irregular menstruation, secondary amenorrhea, gynecomastia, galactorrhea, abnormality result of thyroid function test; acute pancreatitis; hyperamonemia, hyperglycinemia, edema of the extremities.

Alcohol, aspirin, carbamazepine, ducumarol, phenobarbital, barbiturate, pyrimidone, phenytoin, clonazepam, warfarin.
