Indications/Uses
Endocrine disorders: Primary or secondary adrenocortical insufficiency; congenital adrenal hyperplasia, non suppurative thyroiditis & hypercalcemia associated w/ cancer. Rheumatic disorders: Adjunctive therapy for short-term administration in psoriatic arthritis, RA eg, juvenile RA; ankylosing spondyilitis, acute & subacute bursitis, epicondyilitis, synovitis of OA, acute gouty arthritis, acute nonspecific tenosynovitis & post-traumatic OA. Collagen diseases: Maintenance therapy in selected cases of systemic lupus erythematosus, systemic dermatomyositis (polymyositis) & acute rheumatic carditis. Dermatologic diseases: Pemphigus, bullous dermatitis herpetiformis, severe erythema multiforme (Stevens-Johnson syndrome), exfoliative dermatitis, mycosis fungoides, severe psoriasis & severe seborrheic dermatitis. Allergic states: Seasonal or perennial allergic rhinitis, serum sickness, bronchial asthma, drug hypersensitivity reactions, contact dermatitis & atopic dermatitis. Ophth diseases: Severe, acute & chronic allergic & inflammatory conditions involving the eye & its adnexa eg, allergic corneal marginal ulcers, herpes zoster ophthalmicus, anterior segment inflammation, diffuse posterior uveitis & choroiditis, sympathetic ophthalmia, allergic conjunctivitis, keratitis, chorioretinitis, optic neuritis, iritis & iridocyclitis. Resp diseases: Symptomatic sarcoidosis, Loeffler's syndrome not manageable by other means, berylliosis, fulminating or disseminated pulmonary TB when used concurrently w/ appropriate antiTB chemotherapy & aspiration pneumonitis. Hematologic disorders: Idiopathic thrombocytopenic purpura & secondary thrombocytopenia in adults, acquired (autoimmune) hemolytic anemia, erythroblastopenia (RBC anemia) & congenital (erythroid) hypoplastic anemia. Neoplastic diseases: Palliative management of leukemias & lymphomas in adults & acute leukemia of childhood. Edematous states: Induce diuresis or remission of proteinuria in the nephrotic syndrome, w/o uremia of the idiopathic type or that due to lupus erythematosus. GI diseases: Ulcerative colitis & regional enteritis. Nervous system: Acute exacerbations of multiple sclerosis. Miscellaneous: TB meningitis w/ subarachnoid block or impending block when used concurrently w/ appropriate antiTB chemotherapy. Trichinosis w/ neurologic or myocardial involvement.
Dosage/Direction for Use
Initial dosage may vary from 4-48 mg daily. Multiple sclerosis 160 mg daily for a wk followed by 64 mg every other day for 1 mth. Alternate-Day Therapy Regimen: Twice the usual daily dose of corticoid administered every morning, to provide the patient requiring long-term pharmacologic dose treatment w/ the beneficial effects of corticoids eg, pituitary-adrenal suppression, Cushingoid state, corticoid w/drawal symptoms & growth suppression in childn.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Systemic fungal infection; vaccines, immunization. Lactation.
Special Precautions
Prolonged use may produce posterior subcapsular & nuclear cataracts. Stress; ocular herpes simplex, psychotic tendencies; ulcerative colitis, diverticulitis; fresh intestinal anastomoses; active or latent peptic ulcer; renal insufficiency; HTN; osteoporosis; myasthenia gravis. TB. Pregnancy.
Adverse Reactions
Fluid & electrolyte disturbances; muscle weakness, aseptic osteonecrosis, osteoporosis; peptic ulcer w/ perforation, hemorrhage, abdominal distensions; impaired healing; increased IOP; Cushingoid, growth retardation, posterior subcapsular cataracts.
Drug Interactions
Rifampin, anticoagulants, carbamazepine, phenobarb, phenytoin, neuromuscular blockers, aprepitant, fosaprepitant, itraconazole, ketoconazole, diltiazem, ethinylestradiol/norethindrone, cyclosporine, cyclophosphamide, tacrolimus, clarithromycin, erythromycin, troleandomycin, high-dose aspirin. HIV-protease inhibitors; grapefruit juice.
