Indications/Uses
Bronchial asthma. Moderate to severe acute laryngotracheobronchitis (croup) in infants & childn (respules).
Dosage/Direction for Use
Individualized dosage. Turbuhaler Adult 200-1,200 mcg daily in 2-4 divided doses. May be increased to 1,200 mcg in severe asthma. Maintenance: 200-400 mcg twice daily. Respules Adult & childn ≥12 yr 1-2 mg twice daily (morning & evening). Maintenance: 0.5-1 mg twice daily. Childn 3 mth-12 yr 0.5-1 mg twice daily. Acute laryngotracheobronchitis (croup) Infant & childn 2 mg as single administration.
Contraindications
Special Precautions
Pregnancy. Turbuhaler Patient may regain earlier symptoms eg, rhinitis, eczema & pain in muscles & joints during transfer from oral steroid therapy. Temporary increase in oral steroid dose may be necessary. A general, insufficient steroid effect should be suspected if tiredness, headache, nausea & vomiting occurs. Acute exacerbations of asthma may need complementary treatment w/ short oral steroid regimen. Patient should rinse mouth out w/ water after each dose. Patient w/ lung TB; fungal & viral infection in the airways. Initial small but transient reduction in growth in childn & adolescents w/in 1st yr of treatment. Severely compromised liver function. Respules Bronchospasm; HPA axis suppression & adrenal insufficiency, bone density, growth; infections & TB; impaired liver function. Do not use w/ positive pressure delivery systems. Lactation.
Adverse Reactions
Mild irritation in the throat, coughing, hoarseness; Candida infection in the oropharynx; urticaria, rash, dermatitis; bronchospasm; angioedema; anaphylactic reaction; nervousness, restlessness, depression; hypofunction of the adrenal gland, reduction of growth velocity.
Drug Interactions
Turbuhaler Slight inhibition of hepatic metabolism by cimetidine. Respules Ketoconazole & itraconazole.
