Caspofungin acetate
Indications/Uses
In adult & ped patients ≥12 mth for invasive candidiasis including candidemia; esophageal candidiasis; invasive aspergillosis in patients who are refractory to or intolerant of other therapies (ie, amphotericin B, lipid formulations of amphotericin B &/or itraconazole). Empirical therapy for presumed fungal infections in febrile, neutropenic patients.
Dosage/Direction for Use
Administer by slow IV infusion over approx 1 hr. Adult Empirical therapy 70 mg single loading dose on Day 1, then 50 mg daily thereafter. Duration: Fungal infection Min of 14 days; continue for at least 7 days after both neutropenia & clinical symptoms are resolved. Invasive candidiasis 70 mg single loading dose on Day 1, then 50 mg daily thereafter. Continue for at least 14 days after last +ve culture. Esophageal candidiasis 50 mg daily. Invasive aspergillosis 70 mg single loading dose on Day 1, then 50 mg daily thereafter. Childn 12 mth-17 yr 70 mg/m2 single loading dose on Day 1, then 50 mg/m2 daily thereafter.
Contraindications
Special Precautions
Discontinue use if anaphylaxis occurs. History of allergic skin reactions. Concomitant use w/ cyclosporin. Monitor LFTs & evidence of worsening hepatic function. Not to be used during pregnancy & lactation. Neonates & infants <3 mth.
Adverse Reactions
Hypersensitivity reactions. Fever, headache, abdominal pain, pain, chills; nausea, diarrhea, vomiting; elevated liver enzyme levels; increased serum creatinine; anemia (decreased Hb & hematocrit); tachycardia; phlebitis/thrombophlebitis, infused-vein complication, flushing; dyspnea; rash, pruritus; catheter site pain, hypotension.
Drug Interactions
Increased AUC by cyclosporin. Reduced blood conc of tacrolimus. Transient elevated plasma conc w/ rifampin. Efavirenz, nevirapine, phenytoin, dexamethasone, carbamazepine.
