Daptomycin
Indications/Uses
Complicated skin & skin structure infections. Staph aureus bloodstream infections (bacteremia) including those w/ right-sided infective endocarditis, caused by methicillin-susceptible & methicillin-resistant isolates.
Dosage/Direction for Use
Complicated skin & skin structure infections 4 mg/kg by IV infusion in 0.9% NaCl inj over 30 min once every 24 hr for 7-14 days. Renal impairment CrCl ≥30 mL/min 4 mg/kg once every 24 hr, CrCl <30 mL/min including hemodialysis & continuous ambulatory peritoneal dialysis (CAPD) 4 mg/kg once every 48 hr. Staph aureus bloodstream infections (bacteremia) 6 mg/kg IV infusion in 0.9% NaCl inj over 30 min once every 24 hr for 2-6 wk. CrCl ≥30 mL/min 6 mg/kg once every 24 hr, CrCl <30 mL/min including hemodialysis & CAPD 6 mg/kg once every 48 hr.
Contraindications
Special Precautions
Discontinue use if hypersensitivity/anaphylaxis & unexplained signs & symptoms of myopathy occurs. Not indicated for pneumonia & left-sided infective endocarditis due to Staph aureus; prosthetic valve endocarditis. Regularly measure creatinine phosphokinase levels at baseline. Peripheral neuropathy; eosinophilic pneumonia; Clostridium difficile-associated diarrhea; persisting or relapsing Staph aureus bacteremia/endocarditis. Overgrowth of non-susceptible organisms. False prolongation of prothrombin time & elevation of INR. Renal impairment. Pregnancy & lactation. Childn <18 yr. Not to be given to ped patients ≤1 yr.
Adverse Reactions
Anemia; GI & abdominal pain, diarrhea, vomiting, flatulence, bloating & distention, constipation, nausea; asthenia, pyrexia, infusion site reaction; UTI, fungal & candida infection; increased blood creatinine phosphokinase, abnormal LFT; limb pain; dizziness, headache; anxiety, insomnia; pruritus, rash; HTN, hypotension.
Drug Interactions
Tobramycin, warfarin, HMG-CoA reductase inhibitors.
