Cytarabine
Indications/Uses
Induction & maintenance of remission in acute myelocytic leukaemia in adult & childn. Other leukemias eg, acute lymphocytic leukaemia, chronic myelocytic leukaemia (blast phase).
Dosage/Direction for Use
Adult Monotherapy Up to 200 mg/m2 as continuous IV infusion for 5 days repeated at approx 2-wkly intervals. Meningeal leukaemia 30 mg/m2 intrathecally every 4 days until CSF findings are normal, followed by 1 additional treatment. Dose range: 5-75 mg/m2 once daily for 4 days to once every 4 days.
Special Precautions
Anaphylactic reactions. Bone marrow suppression w/ leukopenia, thrombocytopenia & anaemia. Preexisting drug-induced bone marrow suppression. Increased risk of spinal cord toxicity if used intrathecally. Acute pancreatitis; vomiting; conventional & experimental dose schedules. Avoid vaccination w/ live vaccines. Periodic checking of bone marrow exam, liver & kidney functions. Monitor blood uric acid level. Renal or hepatic impairment. Not recommended during pregnancy. Lactation. Neurological AR in juveniles & adolescents.
Adverse Reactions
Sepsis, pneumonia, infection; bone marrow failure, thrombocytopenia, anaemia, megaloblastic anaemia, leukopenia, decreased reticulocyte count; GI disorders; abnormal hepatic function; alopecia, rash; cytarabine syndrome; pyrexia; abnormal bone marrow biopsy & blood smear test. Anorexia; bleeding, thrombophlebitis; skin ulceration.
Drug Interactions
Reduced effectiveness of methotrexate. Decreased steady-state plasma digoxin conc w/ β-acetyldigoxin & chemotherapy regimens containing cyclophosphamide, vincristine & prednisone w/ or w/o cytarabine or procarbazine. Antagonised K pneumoniae strains w/ gentamicin. Potential competitive inhibition uptake of fluorocytosine.
