Meropenem
Indications/Uses
Pneumonia & nosocomial pneumonia, UTI, intra-abdominal infections, gynaecological infections eg, endometritis & pelvic inflammatory disease, bacterial meningitis, septicaemia in adult & childn. Empiric treatment for presumed infections w/ febrile neutropenia, used as monotherapy or in combination w/ antivirals or antifungals.
Dosage/Direction for Use
Adult Pneumonia, UTI, gynaecological infections eg, endometritis 500 mg IV every 8 hr. Hospital acquired pneumonia, peritonitis, presumed infections in febrile neutropenic patient, septicaemia 1 g IV every 8 hr. Meningitis 2 g every 8 hr. Pseudomonas aeruginosa nosocomial infections Adult & adolescent 2 g tds. Childn Infections >50 kg Adult dose, >3 mth-12 yr 10-20 mg/kg every 8 hr. Meningitis 40 mg/kg every 8 hr. Febrile episodes in neutropenic patient 20 mg/kg every 8 hr. Pseudomonas aeruginosa nosocomial infections 40 mg/kg tds.
Special Precautions
Hypersensitivity reactions to β-lactam antibiotics, overgrowth of nonsusceptible organisms, methicillin-resistant staphylococci infections, history of GI complaints, pseudomembranous colitis, CNS disorders, bacterial meningitis, compromised renal function, hepatic disease. Co-administration w/ potentially nephrotoxic drugs. Pregnancy, not to be used in lactation. Childn w/ altered hepatic or renal function. Not recommended in infant <3 mth.
Adverse Reactions
Inj site inflammation, thrombophlebitis, pain; rash, pruritus, urticaria; abdominal pain, nausea, vomiting, diarrhea, pseudomembranous colitis; thrombocythemia, eosinophilia, thrombocytopenia, leucopenia, neutropenia, haemolytic anemia, bleeding; increased bilirubin, transaminases, alkaline phosphatase, lactic dehydrogenase; headache, paresthesia, convulsions; oral & vag candidiasis.
Drug Interactions
Increased plasma conc by probenecid. Reduced valproic acid levels & anticonvulsant effect. Decreased immunological response to the typhoid vaccine.
