Per 50/12.5 mg tab Losartan K 50 mg, hydrochlorothiazide 12.5 mg. Per 100/12.5 mg tab Losartan K 100 mg, hydrochlorothiazide 12.5 mg
HTN for patients in whom combination therapy is appropriate. Reduce risk of stroke in patients w/ HTN & left ventricular hypertrophy.
Dosage/Direction for Use
HTN Usual starting & maintenance dose: 1 tab of Hyzaar 50/12.5 mg once daily, may be increased to 1 tab of Fortzaar once daily or 2 tab of Hyzaar 50/12.5 mg once daily. Max: 1 tab of Fortzaar once daily or 2 tab of Hyzaar 50/12.5 mg once daily. Hypertensive patient w/ left ventricular hypertrophy Usual starting dose: Losartan 50 mg once daily, may be titrated w/ combination of losartan & hydrochlorothiazide 12.5 mg, increased if necessary to losartan 100 mg & hydrochlorothiazide 25 mg once daily.
May be taken with or without food.
Hypersensitivity to losartan, hydrochlorothiazide or other sulfonamide-derived drugs. Anuria. 2nd & 3rd trimesters of pregnancy.
Special Precautions
Hypersensitivity reaction w/ or w/o history of allergy or bronchial asthma. Not indicated for initial therapy. Not to be initiated in patients w/ intravascular vol-depletion (eg, those treated w/ high-dose diuretics). Angioedema. Changes in renal function including renal failure may occur. Bilateral renal artery stenosis or artery stenosis to solitary kidney. Hypotension & electrolyte/fluid imbalance; impair glucose tolerance, decrease urinary Ca excretion; intermittent & slight increases in serum Ca; increase cholesterol & triglyceride levels, precipitate hyperuricemia &/or gout. Exacerbate or activate SLE. Black patients. Not recommended for hepatic impairment or severe renal impairment (CrCl ≤30 mL/min). Lactation. Childn.
Adverse Reactions
Adverse experiences limited to those previously reported w/ losartan K &/or hydrochlorothiazide. Dizziness, asthenia/fatigue, vertigo. Anaphylactic reactions, angioedema, vasculitis including Henoch-Schonlein purpura (rarely), hepatits (rarely), diarrhea, cough, urticaria, erythroderma.
Drug Interactions
Lithium salts, NSAIDs. Losartan: Rifampin, fluconazole, K-sparing diuretics, K supplements or K-containing salt substitutes. Hydrochlorothiazide: Alcohol, barbiturates, narcotics, antidiabetic drugs (oral agents & insulin), other antihypertensives, cholestyramine, colestipol, corticosteroids, ACTH, pressor amines (eg, adrenaline), skeletal muscle relaxants, nondepolarizing (eg, tubocurarine).
