Paliperidone
Indications/Uses
Schizophrenia in adolescents ≥15 yr including acute treatment & recurrence prevention. Acute exacerbations of schizoaffective disorder as monotherapy & in combination w/ antidepressants &/or mood stabilizers.
Dosage/Direction for Use
Schizophrenia Adult ≥18 yr 6 mg once daily. Max: 12 mg daily. Recommended range: 3-12 mg once daily. Adolescent 15-17 yr 3 mg once daily, <51 kg Max: 6 mg daily, ≥51 kg Max: 12 mg daily. Schizoaffective disorder 6 mg once daily. Max: 12 mg daily. Recommended range: 3-12 mg once daily. Mild renal impairment (CrCl ≥50 to <80 mL/min) Initially 3 mg once daily, may be increased to 6 mg once daily. Moderate to severe renal impairment (CrCl ≥10 to <50 mL/min) Max: 3 mg once daily.
Administration
May be taken with or without food: Take consistently either always w/ or always w/o food. Swallow whole, do not divide/chew/crush.
Contraindications
Hypersensitivity to paliperidone or risperidone.
Special Precautions
Discontinue use if neuroleptic malignant syndrome or tardive dyskinesia/extrapyramidal symptoms occur. History of cardiac arrhythmias, congenital long QT syndrome; hyperglycemia & DM; wt gain; known CV or cerebrovascular disease or conditions predisposing to hypotension; seizures or conditions lowering seizure threshold; preexisting severe GI narrowing or dysphagia or significant difficulty in swallowing; clinically significant low WBC or drug-induced leukopenia/neutropenia; risk factors for venous thromboembolism; Parkinson's disease & dementia w/ Lewy bodies; priapism. Intraoperative floppy iris syndrome. Antiemetic effect. Conditions that contribute to core body temp elevation or dehydration. Concomitant use w/ risperidone; psychostimulants eg, methylphenidate; drugs prolonging QT interval. May affect ability to drive & use machines. Severe hepatic impairment. Pregnancy. Not to be used during lactation. Adolescents & childn <12 yr w/ schizophrenia; adolescents & childn <18 yr w/ schizoaffective disorder. Elderly w/ dementia.
Adverse Reactions
Headache, akathisia, somnolence, tremor, cogwheel rigidity; insomnia; vomiting, nausea.
Drug Interactions
Antagonized effect of l-dopa & other dopamine agonists. Decreased mean steady-state Cmax & AUC w/ carbamazepine. Increased Cmax & AUC w/ divalproex Na. Emergence of extrapyramidal symptoms w/ psychostimulants eg, methylphenidate. Drugs prolonging QT interval; centrally-acting drugs, alcohol.
