Isosorbide dinitrate
Prophylaxis of angina pectoris & adjunctive therapy in CHF.
Dosage/Direction for Use
30-120 mg daily in divided doses, increase dose gradually up to max: 240 mg daily in divided doses.
Should be taken on an empty stomach: Take 30 min before meals.
Hypersensitivity to isosorbide dinitrate or to organic nitrate drugs. Hypotension or uncorrected hypovolaemia, severe anemia or arterial hypoxaemia. Constrictive pericarditis & pericardial tamponade, intracranial HTN.
Special Precautions
Patients w/ hypothyroidism, malnutrition, hypothermia, low or normal pulmonary capillary wedge pressure. Patients predisposed to closed-angle glaucoma. Development of tolerance. Alcohol use. Concomitant use w/ BP-lowering drugs. Severe renal or hepatic disease. Pregnancy & lactation. Not recommended in childn.
Adverse Reactions
Headache; cerebral ischemia associated w/ postural hypotension (eg, weakness, transient episodes of dizziness); cutaneous vasodilatation w/ flushing.
Drug Interactions
Physiological antagonism to noradrenaline (norepinephrine), acetylcholine, histamine, other agents. Potentiated hypotensive effects by phosphodiesterase inhibitors (eg, sildenafil, tadalafil, vardenafil). Orthostatic hypotension w/ Ca-channel blockers, antihypertensive agents, phenothiazines, TCAs. Severe hypotension & collapse w/ alcohol.
