Per mL Bupivacaine HCl 5 mg, adrenaline 5 mcg

Indications/Uses

Surgical anesth: Epidural block for surgery & field block (minor & major nerve blocks & infiltration). Analgesia: Continuous epidural infusion or intermittent bolus epidural administration for analgesia in post-op or labour pain & field block (minor nerve block & infiltration).

Overdosage

Contraindications

Hypersensitivity to amide-type local anesth. IV regional anesth (Bier's block); epidural anesth in patients w/ pronounced hypotension eg, cardiogenic or hypovolemic shock.

Special Precautions

Not to be used as IA continuous infusion. Impaired CV function; 2nd- or 3rd-degree AV block; severe or untreated HTN, thyrotoxicosis, ischemic heart disease, cerebrovascular insufficiency, advanced diabetes & other adrenaline-exacerbated conditions; use in peripheral body parts eg, fingers or body parts having low blood supply. May cause allergic reactions due to Na metabisulphite. Insert IV cannula before local anaesth. Retro- & peribulbar inj, paracervical block. Epidural anaesth can lead to hypotension & bradycardia; reduce risk of such effects by eg, vasopressor inj. Inadvertent intravascular administration. High-dose use. Concomitant use w/ class III antiarrhythmics. May affect ability to drive & use machines. Severe hepatic or renal impairment. Pregnancy & lactation. Elderly.

Adverse Reactions

Hypotension; nausea. Paraesthesia, dizziness; bradycardia; HTN; vomiting; urinary retention.

Drug Interactions

Additive systemic toxic effects w/ other local anaesth or structurally-similar agents ie, class IB anti-arrhythmics. Enhanced adrenaline pressor effect w/ non-selective β-blockers eg, propranolol. Severe cardiac arrhythmias w/ halothane during general anaesth. Marked increased BP w/ TCAs, ergot-type oxytocic drugs. Decreased/inhibited adrenaline pressor effect w/ ohenothiazine & butyrophenone derivatives. Class III anti-arrhythmics eg, amiodarone.
