Memantine HCl
Moderate to severe Alzheimer's disease.
Dosage/Direction for Use
Adult 5 mg daily on 1st wk, 10 mg daily on 2nd wk then 15 mg daily on 3rd wk & 20 mg daily from 4th wk onwards. Max & maintenance dose: 20 mg daily. Elderly >65 yr 20 mg daily. Moderate renal impairment (CrCl 30-49 mL/min) 10 mg daily, may be increased to 20 mg daily. Severe renal impairment (CrCl 5-29 mL/min) 10 mg daily.
May be taken with or without food: Take at the same time each day.
Special Precautions
Epilepsy, history of convulsions or predisposing factors of epilepsy. Monitor patients w/ factors that raises urine pH. Recent MI, uncompensated CHF (NYHA III-IV) or uncontrolled HTN. Avoid concomitant use w/ N-methyl-D-aspartate antagonists eg, amantadine, ketamine or dextromethorphan. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May affect ability to drive & use machines. Severe hepatic impairment. Not to be used during pregnancy & lactation. Not recommended in childn <18 yr.
Adverse Reactions
Drug hypersensitivity; somnolence; dizziness, balance disorders; HTN; dyspnoea; constipation; elevated liver function test; headache.
Drug Interactions
Enhanced effects of l-dopa, dopaminergic agonists, anticholinergics. Reduced effects of barbiturates, neuroleptics. Modified effects of antispasmodic agents, dantrolene or baclofen. Risk of pharmacotoxic psychosis w/ amantadine, ketamine, dextromethorphan. Risk of increased plasma levels w/ cimetidine, ranitidine, procainamide, quinidine, quinine, nicotine. Reduced serum level possibility of hydrochlorothiazide. Increased INR w/ warfarin.
