Meropenem trihydrate
Indications/Uses
Pneumonias & nosocomial pneumonias; UTI; intra-abdominal & gynaecological infections (eg, endometritis & pelvic inflammatory disease); bacterial meningitis; septicaemia in adults & childn. Empiric treatment for presumed infections in patients w/ febrile neutropenia as monotherapy or in combination w/ antiviral or -fungal agents. Monotherapy or in combination w/ other antimicrobial agents for polymicrobial infections.
Dosage/Direction for Use
Adult Pneumonia, UTI, gynaecological infections eg, endometritis 500 mg IV every 8 hr. Hospital-acquired pneumonias, peritonitis, presumed infections in neutropenic patient, septicaemia 1 g IV every 8 hr. Meningitis 2 g every 8 hr. Adult & adolescent Nosocomial infections due to Pseudomonas aeruginosa Up to 2 g tds. Childn >3 mth & up to 12 yr 10-20 mg/kg every 8 hr. Childn >50 kg Adult dose. Nosocomial infections due to Pseudomonas aeruginosa & meningitis 40 mg/kg every 8 hr. Febrile episodes in neutropenic patient 20 mg/kg every 8 hr.
Contraindications
Hypersensitivity to meropenem, carbapenems & other β-lactam antibacterial agents eg, penicillins or cephalosporins.
Special Precautions
Discontinue use if hypersensitivity reactions occur. Antibiotic-associated & pseudomembranous colitis; risk of development of drug-resistant bacteria; seizures. Closely monitor hepatic function during treatment. Positive direct or indirect Coombs' test. Patients w/ restricted Na diet. Not recommended in concomitant use w/ valproic acid/Na valproate/valpromide. May affect ability to drive & use machines. Hepatic disease; renal impairment. Avoid use during pregnancy. Not to be used in lactation. Childn <3 mth. Elderly.
Adverse Reactions
Thrombocythaemia; headache; diarrhoea, vomiting, nausea, abdominal pain; increased transaminases, blood alkaline phosphatase & lactate dehydrogenase; rash, pruritus; inflammation, pain.
Drug Interactions
Increased elimination t½ & plasma conc by probenecid. Decreased valproic acid blood levels. Augmented anticoagulant effects w/ oral anticoagulants.
