Indications/Uses
Urogenital trichomoniasis in both genders; non-specific vaginitis. Intestinal & extraintestinal amoebiasis; giardiasis. Acute ulcerative gingivitis & dental infection. Anaerobically infected leg ulcers & pressure sores. Treatment & prophylaxis of anaerobic infection.
Dosage/Direction for Use
Prophylaxis against anaerobic infections in abdominal (especially colorectal) & gynaecological surgery Adult 400 mg 8 hrly. Childn 7.5 mg/kg 8 hrly. Established anaerobic infection Adult Initially 800 mg, followed by 400 mg 8 hrly. Childn 7.5 mg/kg 8 hrly. Duration: 7 days. Urogenital trichomoniasis Adult & childn >10 yr 200 mg tds for 7 days or 800 mg in the morning & 1,200 mg in the evening for 2 days or 2 g as single dose for 1 day, childn 7-10 yr 100 mg tds for 7 days, 3-7 yr 100 mg bd for 7 days, 1-3 yr 50 mg tds for 7 days. Bacterial vaginosis Adult & childn >10 yr 400 mg bd for 7 days or 2 g as single dose for 1 day. Invasive intestinal disease in susceptible subject Adult & childn >10 yr 800 mg tds for 5 days, 7-10 yr 400 mg tds for 5 days, 3-7 yr 200 mg qds for 5 days, 1-3 yr 200 mg tds for 5 days. Intestinal disease in less susceptible subjects & chronic amoebic hepatitis Adult & childn >10 yr 400 mg tds for 5-10 days, 7-10 yr 200 mg tds for 5-10 days, 3-7 yr 100 mg qds for 5-10 days, 1-3 yr 100 mg tds for 5-10 days. Amoebic liver abscess Duration: 5 days. Symptomless cyst passers Adult & childn >10 yr 400-800 mg tds for 5-10 days, 7-10 yr 200-400 mg tds for 5-10 days, 3-7 yr 100-200 mg qds for 5-10 days, 1-3 yr 100-200 mg tds for 5-10 days. Giardiasis Adult & childn >10 yr 2 g once daily for 3 days, 7-10 yr 1 g once daily for 3 days; 3-7 yr 600-800 mg once daily for 3 days, 1-3 yr 500 mg once daily for 3 days. Acute ulcerative gingivitis Adult & childn >10 yr 200 mg tds for 3 days, 7-10 yr 100 mg tds for 3 days, 3-7 yr 100 mg bd for 3 days, 1-3 yr 50 mg tds for 3 days. Acute dental infection Adult & childn >10 yr 200 mg tds for 3-7 days. Leg ulcers & pressure sores Adult & childn >10 yr 400 mg tds for 7 days.
Administration
Should be taken with food: Take during or after meals. Swallow whole w/ ½ glass of water.
Contraindications
Hypersensitivity to nitroimidazoles. CNS disorders; blood dyscrasias. Concomitant use w/ ethanol. Severe hepatic failure. 1st trimester of pregnancy & lactation.
Special Precautions
Discontinue use if peripheral neuropathy or CNS toxicity develop. Blood dyscrasias or active CNS disease. Haemodialysis. Long-term use (>10 days). Severe hepatic disease. Not recommended during 1st trimester of pregnancy & lactation. Elderly.
Adverse Reactions
GI disturbances eg, nausea & unpleasant metallic taste; nausea accompanied by headache, anorexia & vomiting.
Drug Interactions
Disulfiram-like effect w/ alcohol. Potentiated effect w/ coumarin- or indandione-derivative anticoagulants. Decreased hepatic metabolism w/ cimetidine. Confusion & psychotic reaction w/ disulfiram. Increased lithium conc. Decreased t½ & plasma conc w/ phenobarb. Increased plasma conc of phenytoin. Spuriously low values w/ aspartate aminotransferase assay.
