Indications/Uses
Anaemia associated w/ CKD in dialysis & non-dialysis patients.
Dosage/Direction for Use
Erythropoiesis stimulating agent (ESA)-naive patient not on dialysis 1.2 mcg/kg SC inj once mthly. Alternatively, 0.6 mcg/kg IV or SC inj once every 2 wk. ESA-naive patient on dialysis 0.6 mcg/kg IV or SC inj once every 2 wk, may be increased by 25-50% of previous dose if rate of rise in Hb <1 g/dL over a mth. ESA-treated patient Start Mircera IV or SC inj once mthly or once every 2 wk.
Contraindications
Hypersensitivity. Uncontrolled HTN.
Special Precautions
Discontinue use if pure red cell aplasia is diagnosed; severe cutaneous skin reaction eg, SJS or TEN develops. Fe deficiency & inflammatory disorders; chronic blood loss, bone marrow fibrosis, severe Al overload, folic acid or vit B12 deficiencies & hemolysis. Patients w/ hemoglobinopathies, seizures or platelet level >500 x 109/L. May lead to excessive increase in Hb for non-anaemic persons. Evaluate Fe status & adequately control BP prior to & during treatment. Monitor closely for skin reactions. Not to be switched to Mircera if Ab to erythropoietin is suspected or confirmed. Pregnancy & lactation. Ped <18 yr.
