Busulphan
Indications/Uses
Chronic granulocytic leukaemia. Prolongs remission in polycythaemia vera, particularly in cases w/ marked thrombocytosis. Selected cases of essential thrombocythaemia & myelofibrosis.
Dosage/Direction for Use
Chronic granulocytic leukaemia Induction: 0.06 mg/kg daily, initial max dose: 4 mg. Maintenance: 0.5-2 mg daily. Polycythaemia vera 4-6 mg daily for 4-6 wk. Essential thrombocythaemia & myelofibrosis 2-4 mg daily.
Administration
May be taken with or without food: Take w/ chilled liqd. Swallow whole, do not chew/crush. Ensure adequate fluid intake.
Contraindications
Patients w/ demonstrated resistance to busulphan. Pregnancy (1st trimester) & lactation.
Special Precautions
Monitor full blood count closely throughout treatment; should not be given to patients who have recently received radio- or chemotherapy. Discontinue if lung toxicity develops. Ineffective once blast transformation has occurred. Hyperuricaemia, hyperuricosuria to be corrected prior to starting treatment. Avoid in young or asymptomatic patients w/ polycythaemia vera or essential thrombocythaemia unless absolutely necessary. Ensure adequate contraceptive precautions. Pregnancy.
Adverse Reactions
Dose-related bone marrow depression, hyperpigmentation. High doses: Nausea, vomiting, diarrhoea, oral ulceration, hyperbilirubinaemia, jaundice, hepatic veno-occlusive disease, centrilobular sinusoidal fibrosis w/ hepatocellular atrophy & necrosis, cardiac tamponade in thalassaemia patients, alopecia, ovarian suppression, amenorrhoea w/ menopausal symptoms in pre-menopausal patients, severe & persistent ovarian failure including pubertal failure after administration to young girls & pre-adolescents, sterility & testicular atrophy in males.
Drug Interactions
Itraconazole, thioguanine, phenytoin, cyclophosphamide, cytotoxic agents that cause pulmonary toxicity.
