Adenosine

Indications/Uses

Rapid conversion to a normal sinus rhythm of paroxysmal supraventricular tachycardia including those associated w/ accessory bypass tracts (Wolff-Parkinson-White syndrome). Aid to diagnosis of broad or narrow complex supraventricular tachycardia. Sensitization of intracavitary electrophysiological investigations.

Dosage/Direction for Use

Adult & elderly 1st dose: 3 mg rapid IV bolus over 2 sec. 2nd dose: 6 mg rapid IV bolus if supraventricular tachycardia is not attained w/in 1-2 min. 3rd dose: 12 mg rapid IV bolus if supraventricular tachycardia is not attained w/in 1-2 min.

Overdosage

View Myodeen overdosage for action to be taken in the event of an overdose.

Contraindications

Hypersensitivity. Sick sinus syndrome, 2nd or 3rd degree AV block, chronic obstructive lung disease w/ evidence of bronchospasm (eg, asthma bronchiole), long QT syndrome, severe hypotension, decompensated heart failure.

Special Precautions

Discontinue use if angina, severe bradycardia & hypotension, resp failure, asystole/cardiac arrest occur. Patients w/ left main coronary stenosis, uncorrected hypovolemia, stenotic valvular heart disease, left to right shunt, pericarditis or pericardial effusion, autonomic dysfunction or stenotic carotid artery disease w/ cerebrovascular insufficiency, recent MI, severe heart failure, minor conduction defects (1st degree AV & bundle branch block), atrial fibrillation or flutter, accessory bypass tract, prolonged QT intervals; history of seizures or susceptibility to seizures; bronchospasm. Patients on controlled Na diet. Recent & post heart transplant patients. Not to be administered w/ dipyridamole. Not recommended during pregnancy. Not to be used during lactation. Childn 0-18 yr.

Adverse Reactions

Bradycardia, sinus pause, skipped beats, atrial extrasystoles, AV block, ventricular excitability disorders eg, ventricular extrasystoles, non-sustained ventricular tachycardia; dyspnea; flushing; chest pressure/pain, feeling of thoracic constriction/oppression. Headache, dizziness, lightheadedness; nausea; burning sensation; apprehension.

Drug Interactions

Potentiated action by dipyridamole. Avoid use w/ theophylline & other xanthines; xanthine-containing foods & drinks; drugs impairing cardiac conduction.
