Norepinephrine bitartrate
Indications/Uses
BP control in certain acute hypotensive states eg, pheochromocytomectomy, sympathectomy, poliomyelitis, spinal anaesth, MI, septicemia, blood transfusion, drug reactions. Adjunct in cardiac arrest & profound hypotension.
Dosage/Direction for Use
Initially 2-3 mL/min IV infusion. Maintenance: 0.5-1 mL/min. Duration: Up to 6 days.
Contraindications
Hypersensitivity to noradrenaline tartrate or NaCl, Na metabisulfite, citric acid monohydrate, Na citrate dihydrate. Patients hypotensive from blood vol deficits; mesenteric or peripheral vascular thrombosis; profound hypoxia or hypercarbia. Concomitant use w/ cyclopropane & halothane anaesth.
Special Precautions
Sulfite sensitivity. Occlusive vascular diseases eg, atherosclerosis, arteriosclerosis, diabetic endarteritis, Buerger's disease. Monitor BP & rate of flow frequently to avoid HTN. Avoid catheter tie-in technique; extravasation into tissues. Concomitant use w/ MAOIs, triptyline- or imipramine-type antidepressants. Pregnancy & lactation. Paed patients. Not to be administered into leg veins of elderly.
Adverse Reactions
Anxiety; headache; arrhythmia, bradycardia, stress cardiomyopathy; HTN, peripheral ischaemia including gangrene, plasma vol depletion; dyspnoea; inj site extravasation & necrosis.
Drug Interactions
Severe, prolonged HTN & possible arrhythmia w/ volatile halogenated anaesth, MAOIs, linezolid, TCAs, adrenergic serotoninergic drugs or other cardiac sensitizing agents.
