Indications/Uses
CHD; chronic stable angina pectoris; HTN.
Dosage/Direction for Use
Administration
May be taken with or without food: Swallow whole, do not chew/break tab. Avoid grapefruit juice.
Contraindications
Hypersensitivity. Not to be used in patients w/ CV shock, higher-grade aortic stenosis, unstable angina pectoris, acute MI w/in 1st 4 wk; severe hypotension (systolic BP <90 mmHg) or existing heart muscle weakness; acute angina attacks; secondary prevention of MI; history of GI or oesophageal obstruction or any degree of decreased lumen GI tract diameter; Kock pouch; inflammatory bowel disease or Crohn's disease. Concomitant use w/ rifampicin. Hepatic impairment.
Special Precautions
Severe hypotension (systolic BP <90 mmHg); poor cardiac reserve; malignant HTN, hypovolaemia. Diabetic patients. Avoid grapefruit juice. Concomitant use w/ β-blockers, other antihypertensives, macrolides, anti-HIV PIs, azole antimycotics, nefazodone, fluoxetine, quinupristin/dalfopristin, valproic acid, cimetidine. May affect ability to drive or operate machinery. Hepatic impairment. Pregnancy & lactation. Childn & adolescents <18 yr.
Adverse Reactions
Headache, flushing, heat sensation; oedema, vasodilatation; constipation; feeling unwell.
Drug Interactions
Reduced bioavailability & weakened efficacy w/ rifampicin. Increased exposure w/ macrolides, anti-HIV PIs, azole antimycotics, fluoxetine, nefazodone, quinupristin/dalfopristin, cisapride, valproic acid, cimetidine, diltiazem. Decreased exposure w/ rifampicin, phenytoin, carbamazeine, phenobarb. Increased BP-lowering effect of antihypertensives. Deterioration of heart failure w/ β-receptor blockers. Reduced digoxin clearance. Lowered plasma quinidine levels. Concomitant use w/ tacrolimus. Elevated plasma conc & prolonged action w/ grapefruit juice. Falsely increased spectrophotometric values of urinary vanillymandelic acid.
