Pantoprazole Na
Indications/Uses
Short-term use for symptomatic improvement & healing of GI diseases requiring GI reduction eg, duodenal & gastric ulcer, moderate & severe reflux oesophagitis, Zollinger-Ellison syndrome & other pathological hypersecretory conditions.
Dosage/Direction for Use
Gastric & duodenal ulcer, moderate & severe reflux oesophagitis 40 mg daily. Long-term management of Zollinger-Ellison syndrome & other pathological hypersecretory conditions 80 mg daily. Rapid acid control 2 x 80 mg w/in 1 hr. Severe hepatic impairment Not to exceed 20 mg daily.
Contraindications
Hypersensitivity to pantoprazole Na or substituted benzimidazoles.
Special Precautions
Gastric malignancy. Increased risk of Clostridium difficile-associated diarrhoea; GI infections caused by bacteria eg, Salmonella & Campylobacter; osteoporosis-related hip, wrist or spine fractures. Hypomagnesaemia; subacute cutaneous lupus erythematosus. Vit B12 malabsorption due to long-term therapy (>3 yr). Not recommended w/ HIV PIs eg, atazanavir. Long-term use. Discontinue use at least 5 days prior to chromogranin A measurements. May affect ability to drive & use machines. Severe hepatic impairment. Not to be used during pregnancy. Lactation.
Adverse Reactions
Diarrhoea, headache. Benign fundic gland polyps; inj site thrombophlebitis.
Drug Interactions
Reduced absorption of gastric pH-dependent drugs eg, ketoconazole, itraconazole, posaconazole, erlotinib. Reduced bioavailability of HIV drugs eg, atazanavir. Changes in prothrombin time/INR w/ coumarin anticoagulants eg, phenprocoumon or warfarin. Increased methotrexate levels. Increased systemic exposure w/ CYP2C19 inhibitors eg, fluvoxamine. Reduced plasma conc w/ CYP2C19 & CYP3A4 inducers eg, rifampicin, St. John's wort.
