Indications/Uses
Healing & maintenance of erosive or ulcerative GERD. Healing of duodenal ulcers. Pathological hypersecretory conditions including Zollinger-Ellison syndrome. Symptomatic treatment of moderate to very severe GERD. In combination w/ amoxicillin & clarithromycin for H pylori eradication to reduce risk of duodenal ulcer recurrence.
Dosage/Direction for Use
Adult 10 mg once daily, may be increased to 20 mg once daily. Adult/elderly Healing of erosive or ulcerative GERD 20 mg once daily for 4-8 wk. Reflux esophagitis patient 10 or 20 mg bd for further 8 wk. Patient w/ severe mucosa injury 20 mg bd. GERD maintenance 10 or 20 mg once daily. Reflux esophagitis maintenance 10 mg bd. Zollinger-Ellison syndrome 60 mg once daily, may be titrated upwards to 120 mg daily in divided doses. Symtomatic GERD in patient w/o esophagitis 10 mg once daily. H pylori eradication 3-drug regimen: Pariet 20 mg + amoxicillin 1,000 mg + clarithromycin 500 mg bd for 7 days.
Administration
May be taken with or without food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Pregnancy & lactation.
Special Precautions
History of drug hypersensitivity. Increased risk of osteoporosis-related hip, wrist or spine fractures; C difficile-associated diarrhea. Subacute cutaneous lupus erythematosus; severe hypomagnesaemia; vit B12 deficiency. Perform hematological & biochemical test periodically throughout therapy. Monitor thyroid function during therapy. Ensure adequate intake of vit D & Ca. High doses & long-term use. Discontinue use at least 5 days before CgA measurements. May affect ability to drive & use machines. Hepatic cirrhosis & disorder. Not recommended in childn. Elderly.
Adverse Reactions
Rash, urticaria; erythrocytopenia, leukopenia, leukocytosis, eosinophilia, neutrophilia, lymphopenia; elevated AST, ALT, Al-P. γ-GTP, LDH & total bilirubin; constipation, diarrhea, feeling of enlarged abdomen, benign fundic gland polyps; headache; elevated total cholesterol, triglycerides & BUN, proteinuria, increased blood TSH.
Drug Interactions
Increased blood conc of digoxin & methotrexate. Delayed metabolism & excretion of phenytoin. Decreased AUC w/ Al hydroxide- or Mg hydroxide-containing antacids.
