Furosemide
Indications/Uses
Oedema associated w/ heart failure, including pulmonary edema & w/ renal & hepatic disorders. HTN, alone or w/ other antihypertensives. Management of oliguria due to renal failure or insufficiency.
Dosage/Direction for Use
Initially 20-50 mg IM/IV, may be increased by 20-mg increments given not >2-hrly. Max: 1,500 mg daily. Severe renal impairment Not to exceed 2.5 mg/min. Childn 0.5-1.5 mg/kg up to max: 20 mg daily.
Contraindications
Not to be given in anuria or renal failure  caused by nephrotoxic or hepatotoxic drugs or associated w/ hepatic coma; pre-comatose states associated w/ cirrhosis. Prostatic hyperplasia or impaired micturition. Not to be inj IV at rate >4 mg/min.
Special Precautions
Cross-reactivity w/ other sulfur-containing drugs. Prostatic hyperplasia or impaired micturition. Chronic heart failure & moderate hepatic congestion. Avoid use in patients w/ severe hepatic impairment. Not to be given during pregnancy. Lactation.
Adverse Reactions
Fluid & electrolyte imbalance, headache, hypotension, muscle cramps, dry mouth, thirst, weakness, lethargy, drowsiness, restlessness, oliguria, cardiac arrhythmias & GI disturbances.
Drug Interactions
Enhanced digitalis glycoside toxicity, nephrotoxicity of cephalosporin & NSAIDs, ototoxicity of aminoglycoside & other ototoxic drugs. Increased risk of arrhythmias w/ astemizol, terfenadine, halofantrine, pimozide & sotalol. Enhanced neuromuscular-blocking action of atracurium. Enhanced K-depleting effect by corticosteroids, corticotrophin, salbutamol, carbenoxolone, amphotericin B, reboxetine. Enhanced effect of other antihypertensives. Antagonized antihypertensive effect by corticosteroids, NSAIDs or carbenoxolone. Enhanced orthostatic hypotension by alcohol, barbiturates or opioids. Increased allopurinol & tetracycline toxicity. Toxic blood conc of lithium. Symptomatic hyponatraemia w/ hydrochlorothiazide & carbamazepine.
