Na bicarbonate
Metabolic acidosis & renal calculi caused by uric acid.
Dosage/Direction for Use
Adult & paed Systemic alkaliser in cardiac arrest Initially 1 mEq/kg; 0.5 mEq/kg IV, may be repeated every 10 min of continued arrest. Less urgent forms of metabolic acidosis; urinary alkaliser 2-5 mEq/kg IV infused over 4-8 hr.
Metabolic or resp alkalosis; chloride loss due to vomiting or continuous GI suction & hypocalcaemia.
Special Precautions
Ensure adequate alveolar ventilation during cardiac arrest to control arterial pH.
Adverse Reactions
Belching or swelling of stomach due to release of carbon dioxide, stomach cramps & unusual increase in thirst.
Drug Interactions
Adrenocorticoids, mineralocorticoids, corticotropin, amphetamines, quinidine, androgens, steroids, anabolic, antimuscarinincs, ciprofloxacin, K-depleting diuretics (eg, bumetanide, thiazide, ethacrynic acid, furosemide, indapamine), mecamylamine, methenamine, mexiletine, K supplements, salicylates.
