Indications/Uses
Sepsis; meningitis; early & late disseminated Lyme borreliosis; abdominal infections (peritonitis, biliary & GI tract infection); bones, joints, soft tissue, skin, wound & renal infections; infections in patients w/ impaired defense mechanisms; UTI; resp tract infections particularly pneumonia; ear, nose & throat infections; genital infections including gonorrhea; peri-op prophylaxis of infections.
Dosage/Direction for Use
Adult & childn >12 yr 1-2 g once daily. Severe infection May be increased up to 4 g once daily. Neonate, infant & childn (15 days-12 yr) 20-80 mg/kg once daily. Neonate up to 14 days 20-50 mg/kg once daily, not to exceed 50 mg/kg. Meningitis Infant & childn Initially 100 mg/kg once daily. Max: 4 g once daily. Lyme borreliosis Adult & childn 50 mg/kg once daily for 14 days. Max: 2 g daily. Gonorrhea 250 mg single IM dose. Peri-op prophylaxis 1-2 g single dose 30-90 min pre-op.
Contraindications
Hypersensitivity to lidocaine, ceftriaxone or other cephalosporins, penicillin or other β-lactams. Neonate ≤28 days on treatment w/ Ca-containing IV soln. Hyperbilirubinemic newborns, premature neonates up to 41 wk postmenstrual age.
Special Precautions
Discontinue use in case of severe hypersensitivity; history of hypersensitivity to ceftriaxone, other cephalosporins or other β-lactams. Immune-mediated hemolytic anemia. Clostridium difficile-associated diarrhea. Superinfections. Pancreatitis. Monitor CBC during prolonged treatment. False +ve Coombs' test. Not to be mixed or administered w/ Ca-containing soln or products. May affect ability to drive & use machines. Severe renal & hepatic dysfunction. Pregnancy & lactation. Not to be used in neonates at risk of developing bilirubin encephalopathy.
Adverse Reactions
Eosinophilia, leucopenia, thrombocytopenia; diarrhoea, loose stools; increased hepatic enzyme; rash.
Drug Interactions
Antagonistic effects w/ chloramphenicol. Precipitation w/ Ca-containing diluents eg, Ringer's or Hartmann's soln. Increased risk of bleeding w/ vit K antagonists. Chemical incompatibility w/ IV amsacrine, vancomycin, fluconazole, aminoglycosides.
